Objective:In60cases of esophageal squamous cell carcinoma, we observed the expressionlevel of the nuclear transcription factor (NF-ΚBp65) and early growth response gene-1(egr-1) in our experiment,60cases was treated with intensity modulatedradiotherapy(IMRT), everyone’s recent radiotherapy effect was evaluated by follow upvisit.Analysis the ralationship of the two NF-ΚBp65and EGR-1expression in esophagealsquamous cell carcinoma specimens, then analyze the relationship between the two factorexpression and clinicopathological features in60cases, in addition, investigate relationshipbetween the expression of two factors in esophageal cancer tissue specimens with60patients’ recent radiotherapy effect, to preliminary explore the indicators that we canvaluate recent radiotherapy effect of esophageal cancer and the prognosis of60esophagealcancer patients, then provide new support for the theory which related to individualtreatment of patients with esophageal cancer.Materials and Methods:sixty cases who were with esophageal squamous cell carcinoma added in this task, wepicked up the tumor tissue samples before60members treated intensity modulatedradiotherapy(IMRT), and the type of pathology specimens have been checked to verify thepathology of60patients, all patients had complete clinical information and follow-up data,while taking20cases of biopsy normal esophageal mucosa tissue samples ascontrols,everyone’s recent radiotherapy effect was evaluated by follow up visit,nobodywas out of follow up, while taking20cases of normal esophageal mucosa biopsy tissuesamples as controls, SP immunohistochemical technique was used to detect NF-ΚBp65and EGR-1expression in60cases of tumor tissue specimens. Clinical treatment which wasused to60patients with esophageal squamous cell carcinoma was IMRT mainly conductedthrough6MV-X-ray linear accelerator. Recent radiotherapy effect was mainly based on thethe results of of chest CT examination and esophageal barium at end of radiotherapy and3 months later.Result:1.At60cases of esophageal squamous cell carcinoma, expression of NF-ΚBp65or inthe nucleus and cytoplasm, and the positive expression rate of NF-ΚBp65was60%(36/60),and at20cases of normal esophageal mucosa NF-ΚBp65The positive rate of15%(3/20),through statistical analysis, the two groups was statistically significant difference (χ2=12.158, P <0.05).2.In60cases of esophageal squamous cell carcinoma, EGR-1protein expression wasmainly located in the nucleus, where16patients EGR-1protein expression was positive,its protein expression was26.67%(16/60),in20cases who were with normal esophagealmucosa,there are2members with EGR-1protein positive expression, the positive rate was10.0%,by statistical analysis, the difference between the two groups was no statisticallysignificant (χ2=1.529, P>0.05).3.In esophageal squamous cell carcinoma, NF-ΚBp65expression levels correlatedwith the lymph node metastasis, distant metastasis,clinical staging, depth of invasion ofpatients, and there was a positive correlation with with lymph node metastasis, distantmetastasis,clinical stages, depth of invasion, which was statistically analyzed bySpearman rank correlation, the correlation coefficient were0.266,0.205,0.316,0.276, butNF-ΚBp65expression was no significant correlation with sex,age, the degree ofdifferentiation of patients who have esophageal cancer (P>0.05).4.In60cases of patients with esophageal squamous cell carcinoma specimens,positive expression of EGR-1protein is associated with lymph node metastasis, depth ofinvasion, clinical stage of patients, through statistical Spearman rank correlation analysis,correlation between EGR1-protein expression and these three indicators above wasnegative, the correlation coefficient were-0.300,-0.255,-0.309, but it showed that therewas no correlation between EGR-1protein expression with esophageal cancerdifferentiation, age, gender, distant metastasis of60patients.5.In all patients specimens, it showed that there was a negative correlation betweenNF-ΚBp65and EGR-1protein expression (r=-0.256, P <0.05).6.In60cases, the rate was60%(36/60) that NF-ΚBp65expression positive in allpatients specimens,and the effective rate of radiotherapy was63.89%(23/36), the negativeexpression of NF-KB (p65) was40%(24/60),the radiotherapy effective rate of91.67%(22/24), by statistical analysis, there had statistically significant difference (χ2=5.962, P<0.05), the statistical Spearman rank correlation analysis showed that NF-KB (p65) protein expression in esophageal cancer have negative correlation with recent efficacy of IMRT inesophageal cancer.(r=-0.314, P=0.014, P <0.05)7. The positive expression of EGR-1protein in60cases of esophageal squamous cellcarcinoma was26.67%(16/60),while the negative expression rate was73.33%(44/60),therecent radiotherapy efficacy rates were100%(16/16) and65.91%(29/44), respectively,statistical analysis showed that the difference was statistically significant (χ2=5.568, P<0.05), statistically Spearman rank correlation analysis showed that EGR-1proteinexpression in esophageal cancer of esophageal cancer had positive correlation with recentefficacy of IMRT in esophageal cancer.(r=0.348, P=0.006, P <0.05).Conclusion:1.Compared with normal esophageal mucosa, NF-κB in esophageal squamous cellcarcinoma tissue expression was significantly higher.The expression level in patients ofNF-κB had had closely correlation with lymph node metastasis, distant metastasis,clinicalstage, depth of invasion of patients who were with esophageal cancer, while no significantcorrelation with the extent of the patient’s tissue differentiation, age, sex, suggesting thatNF-κB protein the expression may be associated with esophageal squamous cell carcinomaoccurrence and development, as well as processes such as invasion and metastasis isassociated.2.EGR-1protein expression levels in normal esophageal mucosa were higher than inesophageal squamous cell carcinoma, but had no significant difference. It had nocorrelation that EGR-1protein expressed positive with patients’ degree of differentiation,age,gender and distant metastases, to the contrary,there were closely correlation withlymph node metastasis, depth of invasion, clinical stage of patients that were withesophageal cancer.Suggesting that EGR-1protein expression were colsed with invasionand metastasis of esophageal squamous cell carcinoma.3.In all of esophageal squamous cell carcinoma specimens which we detect,NF-ΚBp65and EGR-1protein expression was negatively correlated (r=-0.256, P <0.05)4.In60cases of esophageal squamous cell carcinoma, NF-KB (p65) proteinexpression had negative correlation with recent radiotherapy efficacy of IMRT inesophageal cancer,while EGR-1protein expression in esophageal cancer were positivelycorrelated with recent radiotherapy efficacy of IMRT in esophageal cancer. Esophagealsquamous cell carcinoma patients who were with NF-KB (p65) protein highly expressedoften had poor results of recent radiotherapy efficacy, but patients who were with EGR-1protein high levels expression often had better results of recent radiotherapy efficacy, suggesting that these two factors can be predicted as recently esophageal cancerradiotherapy efficacy indicators, clinicians can according to different express levels of thetwo factors in different patients, draw up individualized treatment programs.
