Objective: Observation the clinical efficacy of stomach topicalpaste in the abdominal cavity+TDP lamp irradiation of the patients withspleen deficiency type of ulcer, and the effect on the serum levels ofgastrin, somatostatin, nitric oxide and nitric oxide synthase. The objectiveof the study is to investigate the mechanism of protective effects to thegastric mucosa of stomach topical paste+TDP lamp irradiation on thepatients with spleen deficiency type of ulcer. Methods: The randomizedcontroll method is adopted in this test,42cases of the patients with spleendeficiency type of ulcer were randomly divided into two groups,21casesin the control group are given the omeprazole injection42.6mg+0.9%NS250ml, intravenous infusion,1time/day; And the aluminummagnesium carbonate tablets,1g/time,3times a day, Juefu half an hourafter the meal, for two weeks. After that, changing intravenous infusionomeprazole to oral omeprazole enteric-coated capsules,20mg/time,2times a day, taking half an hour before a meal; Meanwhile, aluminummagnesium carbonate tablets are continued to be take, treatment for twoweeks. In the method based on the control group, the treatment group areaccepted the stomach topical paste in the abdominal cavity+TDP lampirradiation, the treatment is for four week. Observation and comparisonthe clinical efficacy of two groups before and after treatment, and thechanges of expression levels of serum gastrin, somatostatin, nitric oxide(NO), nitric oxide synthase (NOS). Results: TCM syndromescomparison:21cases of the treatment group, there were10casesmarkedly effective, effective in10cases, ineffective in1patients. Theeffective rate was48%, the total effective rate was95%;21cases in the control group, there were5cases markedly effective, effective in10cases,ineffective in6patients. The effective rate was24%, the total effectiverate was71%; there was a significant difference (P<0.05). The TCMsyndromes efficacy of the treatment group is better than the control group.Comparison the results of gastroscopy:21cases of the treatment group,there were6cases reach the clinical cure, there were10cases markedlyeffective, effective in4cases, ineffective in1patients;21cases in thecontrol group, there were1cases reach the clinical cure, there were7cases markedly effective, effective in11cases, ineffective in2patients;there was a significant difference in patients with gastroscopy results(P<0.05). The gastroscopy results of the treatment group is better than thecontrol group. Comparison the expression levels of serum gastrin,somatostatin, nitric oxide, nitric oxide synthase of the two groups: Aftertwo weeks of the treatment, the expression levels of serum gastrin andsomatostatin were decreased in both groups, however,there was asignificant difference in the two groups(P <0.05). The treatment grouphad a more pronounced decline. The expression levels of nitric oxide andnitric oxide synthase were increased in both groups, however,there was asignificant difference in the two groups(P <0.05). The treatment groupincreased more than the untreated group. Conclusions: This clinicalstudy showed that stomach topical paste in the abdominal cavity+TDPlamp irradiation of the patients with spleen deficiency type of ulcer has aexactly clinical efficacy, it can significantly improve the clinicalsymptoms and signs of the patients with spleen deficiency type of ulcer,and it can also reduce the expression levels of serum gastrin andsomatostatin, increasing the expression levels of nitric oxide and nitricoxide synthase. We believe that the mechanism of protective effects tothe gastric mucosa of stomach topical paste is may be related to theregulating of the expression levels of serum gastrin, somatostatin, nitric oxide, nitric oxide synthase. And we also believe that stomach topicalpaste can regulate gastrointestinal hormone secretion, regulatingneurotransmitter release, improving gastric mucosal blood flow, therebymaintaining the integrity of the gastric mucosal barrier, and ultimatelyachieve the purpose of promoting ulcer healing. At last, stomach topicalpaste in the abdominal cavity+TDP lamp irradiation of the patients withspleen deficiency type of ulcer has a exactly clinical efficacy, it isdeserved to use in clinical.
