Objective: Study the concentration of cerebrospinal fluid lactic acid(CSFL) after aneurysm clipping with subarachnoid haemorrhage amount, Hunt- Hess classification, GCS score, GOS score and the relationship between the occurrence of complications, explore CSFL concentration’s relationship with the prognosis of patients with aneurysms after clipping.Methods: By using blood gas analyzer test the concentration of blood and CSFL at at the same time in patients with intracranial aneurysms after clipping craniotomy, as well as to the patients with postoperative follow-up of 6 months, the parallel GOS score.Statistical analysis CSFL concentration with subarachnoid haemorrhage amount, the relationship between intracranial pressure, GCS score, GOS score, and observed if there is a correlation in the cerebrospinal fluid and blood lactic acid concentration, give a postoperative dynamic review head CT, observe the presence of hydrocephalus and cerebral infarction,and according to the concentration of CSFL grouping, statistical group differences of complications.Results: 1,The Pearson correlation coefficient between CSFL concentration and subarachnoid haemorrhage amount for(r = 0.638, P = 0.000 < 0.001(bilateral test));2,The Pearson correlation coefficient between CSFL concentration and GOS score for(r = 0.794, P = 0.000 < 0.001(double side));3, The Pearson correlation coefficient between CSFL concentration and GCS score for(r = 0.752, P = 0.000< 0.001(double side));4,The Pearson correlation coefficient between CSFL concentration and Hunt- Hess classification for(r = 0.694, P = 0.000 < 0.001(double side));5,CSFL concentration of no complications is(2.09 + 0.47)mmol/L, complications group is(3.52 + 1.17) mmol/L, t test between the two groups(P = 0.000 < 0.001),there were significant differences;6,Blood lactic acid concentration changes associated with CSFL concentration, but the blood lactic acid density in the normal range; 7,When CSFL concentration > 3.0 mmol/ L, complications, and a worse prognosis significantly.Conclusion:1, Cerebrospinal fluid lactate concentration is significantly associated with subarachnoid haemorrhage amount, GOS, Hunt- Hess classification, GCS score, and the occurrence of complications after aneurysm rupture hemorrhage, the higher the FISHER grading, the higher Hunt- Hess classification, the higher the concentration of cerebrospinal fluid lactic acid, the prognosis more worse.CSFL can be used as a reference indicator of the prognosis of aneurysms early postoperative evaluation; 2,When CSFL concentrations moderately elevated(≤3.0mmol/L), the prognosis has no obvious statistical significance, CSFL concentration increased significantly(> 3.0mmol/L), prognosis significantly worse.
