Clinical Study On Relationship Between FOXP3mRNA、RORγtmRNA, Relate Immune Factors And Unexplained Recurrent Spontaneous Abortion

Objective: To investigate the change of FOXP3 mRNA,RORγtmRNA, IL-6, IL-17, TGF-β1, CCL17, CCR4 in women with unexplained recurrent spontaneous abortion(URSA) and its clinical significance.Methods:Collected 50 healthy non pregnant women(group HNP)、50 healthy pregnant women(group HEP)and 50 unexplained recurrent spontaneous abortion of pregnant women(URSA group) from May 2013 to February 2015 in the central hospital of Huangshi,Maternal and child health hospital of Huangshi,Huazhong fu kang infertility hospital of Huangshi. Then,test 3 groups relative expression levels of FOXP3 m RNA, RORγtm RNA in peripheral blood mononuclear by Real-time PCR,and concentrations of serum IL-6,IL-17,TGF-β1,CCL17,CCR4 by enzyme-linked immune sorbent assay(ELISA).Results: 1.The relative expression of FOXP3 m RNA, RORγtm RNA in HNP Group, HEP group and URSA group:HEP group Foxp3 m RNA expression level(30.26 ± 2.92) higher than URSA group(6.50 ± 1.92) and HNP(5.43 ± 1.48), P<0.05;URSA group Foxp3 m RNA expression level(6.50 ± 1.92) higher than HNP group(5.431.48), P<0.05。URSA group RORγt m RNA expression level(26.72 ± 2.86) higher than HEP group(10.93 ± 2.14) and HNP(4.56 ± 1.08),P < 0.05。 HEP group RORγtm RNA expression level(10.93 ± 2.14) higher than HNP group(4.56 ± 1.08),P < 0.05。HEP group Foxp3 m RNA/RORγt m RNA(3.00 ± 1.53) higher than URSA group(0.25 ± 0.06) and HNP(1.28 ± 0.39),P < 0.05; HNP group Foxp3 m RNA/RORγt m RNA(1.280.39) higher than URSA group(0.25 ± 0.06),P < 0.05 。 2.The concentrations of IL-6, IL-17, TGF-β1, CCL17 and CCR4 in HNP Group, HEP group and URSA group:HEP group[TGF-β1(2547.59 ± 349.62)、CCL17(944.00 ± 176.10)、CCR4(320.36 ± 36.96)] ng/L higher then URSA group[(970.68 ± 325.61) 、(404.20 ± 104.48)、(125.50 ± 25.94)] ng/Land HNP group[(699.27 ± 131.12)、(268.61 ± 27.63)、(78.21 ± 12.28)] ng/L, P < 0.05; URSA group [ TGF-β1( 970.68 ± 325.61)、CCL17(404.20 ± 104.48)、CCR4(125.50 ± 25.94)] ng/L higher then HNPgroup[(699.27 ± 131.12)、(268.61 ± 27.63)、(78.21 ± 12.28)]ng/L,P < 0.05。URSA group[IL-6(29.53 ± 3.14),IL-17(62.45 ± 7.44)] ng/L higher then HEP group [(16.59 ± 2.67),(37.42 ± 4.02)] ng/L and HNP group [(10.18 ± 2.76),(25.47 ± 6.15)] ng/L,P < 0.05; HEP group [IL-6(16.59 ± 2.67), IL-17(37.42 ± 4.02)] ng/L higher then HNP group [(10.18 ± 2.76),(25.47 ± 6.15)] ng/L, P < 0.05。3.Relationship between the expression of FOXP3 m RNA、RORγtm RNA、IL-6、 IL-17、 TGF-β1,、CCL17、CCR4 and number of abortions in URSA group:The relative expression of Foxp3 m RNA(5.38 ± 0.57) and the concentrations of [TGF-β1(648.66 ± 257.61)、CCL17(322.28 ± 114.24)、CCR4(102.50 ± 25.32)] ng/L in abortion 3 times or more of URSA were lower than 2 times(( 7.09 ± 0.35)、( 1203.86 ± 63.62)、(463.52 ± 33.19)、(142.17 ± 5.93)]ng/L),P<0.05。 4.The correlation analysis of FOXP3 m RNA, RORγtm RNA,serum IL-6, IL-17, TGF-β1, CCL17 and CCR4 :In the URSA group, the relative expression of Foxp3 m RNA is negative relevant to RORγtm RNA and IL-6(r =-0.764, P < 0.05; r =-0.799, P < 0.05); the concentration of IL- 17 is positively relevant to IL-6 and RORγtm RNA(r = 0.75, p < 0.05; r = 0.651, p < 0.05), the concentration of CCR4 is positively relevant to CCL17 and Foxp3 m RNA(r = 0.76, p < 0.05; r = 0.67, p < 0.05), the relative expression of Foxp3 m RNA is positively relevant to TGF-β1(r = 0.453, p < 0.05).Conclusion: 1.In the URSA group, Foxp3 m RNA,which is specific transcription factor of Treg cells(Related to the immune tolerance induction),is express abate;but,RORγtm RNA,which is specific transcription factor of Th17 cells(Closely associated with autoimmune inflammatory lesions),is express to enhance.Prompt Foxp3 m RNA lower expression and RORγtm RNA high expression is associated with the onset of spontaneous abortion; Meanwhile,the Foxp3 m RNA/RORγt m RNA of abortion patients decreased obviously,is indirect sign of the imbalances of Treg cells and Treg cells disturbed the dynamic balance of maternal-fetal immune tolerance system, may be one of the important causes of URSA. 2.In the URSA group,the enhanced expression of IL- 6, IL- 17; Anormaly expression of CCR4、CCL17、TGF-β1;these changes disturbed the dynamic balance of maternal-fetal immune tolerance system, may be one of the important causes of URSA.
