Objective 1、To analyze the effects of factors for cognitive impairment after first episode cerebral infarction.2、To explore the incidence of cognitive impairment of the patients with different parts of the lations of first episode cerebral infarction and the characteristics of the damaged cognitive domain,to observe the relationship between the parts of the lesions and cognitive impairment.3、To explore the relationship between the responsibility artery after first episode cerebral infarction with cognitive impairment. Methods Collected from February 2013 to October 2014 our hospital neurology treated 206 cases of first episode patients with cerebral infarction. According to the parts of the lesions,206 patients were divided into 28 cases of the frontal lobe,24 cases of temporal lobe,23 cases of parietal lobe,5 cases of occipital lobe,73 cases of basal ganglia,14 cases of thalamus,20 cases of cerebellum and 19 cases of brainstem. According to the responsibility artery,they are divided into 18 cases of anterior cerebral artery(ACA)group,109 cases of middle cerebral artery(MCA)group,15 cases of anterior choroidal artery(ACh A)group,25 cases of posterior cerebral artery(PCA)group and 39 cases of vertebral basilar artery(VBA)group. Using the Chinese version of the Montreal Cognitive Assessment scale(Mo CA)for patients with cognitive function. Record all the cases of the gender,age,education,smoking,drinking,imaging data,hypertension,type 2 diabetes. Taking peripheral venous blood to check blood glucose,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL – C)and high density lipoprotein cholesterol(HDL – C)on the second day early in the morning on an empty stomach. Results 1、Cognitive impairment group and non cognitive impairment group had no significant difference in gender,age,education,smoking,drinking,hypertension,TC,TG,LDL-C and HDL-C(P > 0.05);Cognitive impairment group was significantly higher than non cognitive impairment group in type 2 diabetes(P<0.05).2 、 The incidence of cognitive impairment after first episode cerebral infarction parts had significant difference with parts of the lations(P<0.05),the frontal lobe group had the highest incidence of cognitive impairment,above 92%;followed by the thalamus group,more than 85%;again for the temporal lobe group,more than 75%;the cerebellum and brainstem group had the lowest incidence,about 30%.3、Frontal lobe group was lower than other groups in visuospatial and executive function and attention(P<0.05);thalamus group was lower than other groups in visuospatial and executive function 、memory、attention、language and orientation(P<0.05);temporal lobe group was lower than other groups in naming and memory(P<0.05); three groups of Mo CA scores were lower than other groups and had significant difference(P<0.05).4 、 The incidence of cognitive impairment after first episode cerebral infarction had significant difference with the blocking responsibility artery(P<0.05),the cerebral infarction of MCA blood area had the highest incidence of cognitive impairment,more than 70%. Conclusions 1、Type 2 diabetes is one of the effects of factors of cognitive impairment after first episode cerebral infarction;2、Cognitive impairment after first episode cerebral infarction is relative to parts of the lations;the damaged cognitive domain of the different parts of the lations of cerebral infarction is also different.3、Cognitive impairment after first episode cerebral infarction has different incidence with the blocking responsibility artery,the cerebral infarction of MCA blood area has the highest incidence of cognitive impairment.
