Objective:To analyze the etiology and risk factors of acute pancreatitis with acute kidney injury(AKI) in order to provide the basis for a better understanding and prevention of AKI..Methods:A retrospective analysis 285 cases of acute pancreatitis,which include with AKI patients 44 cases of general information, possible risk factors, clinical features, laboratory examination and prognosis, and merge into AP AKI group (AKI group) and not merge AKI group (Non-AKI group), to analyze the relationship with prognosis. Results Patients enrolled in the study of AKI group have significant differences for the incidence of gender (male), hypertension, heart failure, hypoalbuminemia RRT, mechanical ventilationis, the use of vasoactive drugs are higher than the control group.While it has lower survival rate than the control group. The incidence of AKI was no significant difference between the different stages. Univariate analysis found that the age, heart failure, hypoalbumin emia, COPD, fever, bacteremia, surgery, electrolyte imbalance, cardiac arrhythmias, MODS, mechanical ventilationis are induced AP merger AKI risk factors (p<0.01).In which have higher incidence of MODS, higher level of BUN and SCr(p<0.01)compared with the control group. The incidence of renal replacement therapy and patient survival compared with control group was not statistically significant. Blood urea nitrogen and serum creatinine early prediction of survival in patients with AP merger AKI was statistically significant (P<0.05).Conclusion The incidence of AKI and mortality were high in AP patients,.Heart failure, mechanical ventilation, surgery, MODS, hypoalbuminemia is a serious risk factor for death in patients with AP merger AKI.
