Objective:Tosystematicly evaluate the clinical efficacy of the central Pancreatec-tomy and distal Pancreatectomy. Methods:Using ComPuter to research database such as: China journal full text database CNKI owned by CNKI (1988.1-2014.3); the National Library of Medicine (NLM) under the PubMed (1988.1-2014.3), Holland (medical abstract database EMBASE1988.1-2014.3), the National Library of medicine MEDLINE (1988.1-2014.3), American Science Citation Index SCI (1988.12014.3) The Cochrane Library library and Cochran (1988.1- 2014.3), Central Register of controlled trials, the number of degree thesis database library and conference PaPers database. To evaluate the 15 included controlled clinical trials using RevMa5.2 software. Results:1079 cases of Patients were included in our study,which consisted of 436 CP cases and 643 DP cases. Meta-analyses showed that ComPared CP with DP,the results of Postoperative Pancreatic fistula were 38.3% and 24%[OR:2.0,95%CI:1.48-2.71,P<0.01],PostoPerative comPlicationswer 45.7% and 31.7%[OR:1.52,95% CI:1.11~2.08,P<0.05],PostoPerative endocrine insufficiency rePorted were 6% and 24%.The total death rate was 0.9%.ComPared with DP,CP have a higher death rate and Pancreatic fistularate,but have a lower endocrine insufficiency rate (OR:0.21,95%CI:0.06-0.78,P<0.05) and exocrine insufficiency rate(OR:0.49,95% CI:0.27~0.78,P<0.05).There is statistical significance between oPerating time[SMD:59.23,95%CI:22.41-96.05,P<0.01].Conclusion:These findings indicate that although the central Pancreatectomy has a high PostoPerative comPlication incidence, it can be accePted clinically, which may be a reasonable oPeration method to Preserve Pancreatic exocrine and endocrine function.
