Distal pancreatectomy have been a common operation style of pancreatic tumor,chronical pancreatitis and pancreatic trauma.As the developing of surgical technique,the morbidity of distal pancreatectomy is decreasing,but the effect is still not satisfied.So how to prevent and treat postoperative complications is very important for the maneuver.To further discuss the risk factors of these complications,retrospective analysis of risk factors with complications performed for DP in our hospital.MethodsPatients who underwent DP at the second affiliated hospital of Zhejiang University from January 2001 to March 2009 were enrolled.The postoperative complications including pancreatic leakage,abdominal infection,pneumonia,pulmonary embolism,wound infection,renal failure,delaying of gastric emptying were analysed.Major risk factors were divided into three categories:(1)patient-related factors,such as age,gender,the albumin level of serum;(2)operation-related factors,such as operation style,operative time,estimated blood loss and blood transfusions during operation;and(3)disease-related factors,such as the size,characters and the position of tumor.Statistics were managed with the SPSS software v12.0 to compare the correlation between postoperative complications and major risk factors. ResultsOf the 94 patients undergoing distal pancreatectomy,overall postoperative major complications was 16%(15/94) including pancreatic leakage was 9.6%(9/94),and in four of it combining abdominal infection 1,pneumonia 1,wound infection 1,disfunction of gastric emptying 1 respectively;The sigle-factor analysis show that the postoperative complications rate have a highly significant corrlation with operative time,intraoperative blood loss and intraoperative blood transfusion(p<0.05).ConclusionsThe complications of DP have a certain corrlation with operative time,intraoperative blood loss and intraoperative blood transfusion,so surgical improvement of technique is very important for preventing the morbidity.
