Effectiveness Of Diabetes Management Based On Clinical Pathway In Communities

Objective:To evaluate the effectiveness of the clinical pathway management for type 2 diabetes applied to communityMethod:Self-control clinical trial was applied to the present study. The intervention measure based on the’clinical pathway’ idea was employed to community management of diabetes, in order to realize standardization and informationization for diabetes management and prevention and control of diabetes complications. The control level of main diabetes management indexes such as fasting blood glucose,HbAlc and others was used to evaluate the effectiveness in the study.Result:The mean fasting blood glucose and systolic blood pressure in 2012 were found to decrease as compared with that in 2011,being reduced by 0.28mmol/L (p<0.05) and 1.58mmHg (p< 0.05),respectively. But the mean diastolic blood pressure in 2012 was higher by 1.25mmHg (p<0.05) than that in 2011.Stratified analysis showed the means of the fasting blood glucose and the HbA1c among the patient with higher HbA1c than normal in 2012 were statistical significantly lower than that in 2011, lower by 1.49mmol/L(p<0.05) and 1.22%(p<0.05) respectively.Among the patients with normal HbAlc, however, the means of the fasting blood glucose and the HbAlc in 2012 were statistical significantly higher than that in 2011,higher by 0.38mmol/L (p<0.05) and 0.63%(p<0.05) respectively. The rest of the indicators did not have statistical significance for two years (p> 0.05).Conclusion:Clinical pathway plays a positive role for standard management of diabetes and cost control in the community, which provides important assistance for GPs in their day-to-day health management for diabetes.
