Objective:To observe the clinical efficacy of Da Ming Yin decoction on treatment of diabetic macular edema (DME) (stndrome of deficiency of both Qi and Yin, blood stasis blockingcollaterals),and to explore the mechanism of treatment of DMEMethods:The 26 patients (40 eyes) with DME were randomly divided into treatment group 14 patients (20 eyes) treated with Da Ming Yin and control group 12 patients (20 eyes) treated with calcium dobesilate. After 3 months later, to observe before and after treatment on visual aeuity, fundus and fundus fluorescein angiography(FFA), central macular thickness(CMT), blood glucose, blood pressure, serum lipid, Chinese medical syndrome pattern, compare the data with statistical analysis.Results:(1) The total curative effective rate:After the treatment, the total curative effective rate in treatment group was 90%, superior to that in the control group (75%). The differences of efficacy had statistical significance (P<0.05).(2)Visions:The visions of the both groups were improved. The differences of efficacy had statistical significance (P<0.01); The total curative effective rate in treatment group was superior to that in the control group.After the treatment, the differences of visions between two groups had statistical significance. (P<0.05)(3) Fundus and FFA:The results on fundus exami nation and fundus fluorescein angiography of the treatment group were superior to that of the control group. The differences of visions between two groups had statistical significance (P<0.05).(4) Central macular thickness(CMT):Compared with the before treatment, optical coherence tomography demonstrated that the central macular thickness (CMT) of the two groups had reduced. The differences of efficacy had statistical significance. (P<0.05)(5) Blood glucose, blood pressure, serum lip id:The blood glucose and blood pressure of both groups had no apparent improvements after the treatment. There was no statistical significance (P>0.05).The results pointed out that both of the groups had significant differences on blood glucose and blood pressure. Compared with before treatment, the contents of triglyceride (TG), total cholesterol (TC) and low-density lipoproteincholesterol(LDL-C) of two groups had all reduced. The differences of efficacy had statistical significance (P<0.05) and the control group was superior to the treatment group.(6) The Chinese medical syndrome pattern:After the treatment, the treatment group had obvious improvements of TCM syndrome. The total effective rate of the TCM syndrome was 92.6% superior to the control group. The differences of efficacy had statistical significance (P<0.05).Conclusion:The traditional Chinese medicine Da Ming Yin had significant curative effect on the treatment of diabetic macular edema (DME) (stndrome of deficiency of both Qi and Yin, blood stasis blocking collaterals) superior to the Caleium dobesilate. Da Ming Yin benefit primordial Qi strengthen spleen and nourishing Yin. Da Ming Yin removes blood stasis, alleviates water activating and improves eyesight, treating both symptoms and root causes. Da Ming Yin promotes blood to resolve stagnation, promotes blood circulation to resolve stasis, both specimens, and adjust the content of vascular endothelial growth factor. The possible mechanism of action is by adjust viscera, improved blood circulation regulating functional activities of qi, to improved the visual function;By improving the microcirculation and blood sugar and lipid metabolism as a result of high coagulation state, reduce capillary permeability,copair endothelial function,control and impre diabetic macular edema,improve visual function.
