Objective: To study the relationship between the spermatozoon factor and fertilization rate, cleavage rate, good quality embryo rate, good quality blastocyst rate and pregnancy rate and to provide reference for choosing the different methods of fertilization and male treatment.Methods:The patients undergoing the IVF/ICSI treatment cycles in the reproductive health hospitals of Nan hua xing hui from June 2011 to June 2014 were included in this retrospective study. All patients were treated by long protocol controlled ovarian hyperstimulation. The fresh ejaculation semen were used in the IVF(642)/ICSI(85) cycles. 1. The 580 IVF cycles were divided into teratospermia group(normal sperm morphology <4%) and normal sperm morphology group(normal sperm morphology ≥4%) by sperm morphology. The outcomes were compared the teratospermia group with normal sperm morphology group. The outcomes refer to fertilization rate, cleavage rate, good quality embryo rate, good quality blastocyst rate and pregnancy rate. The 85 ICSI cycles were divided into severe teratospermia group(normal sperm morphology <2%), mildly teratospermia group(2%≤normal sperm morphology<4%) and normal sperm morphology group(normal sperm morphology≥4%) by sperm morphology. The outcomes which refer to fertilization rate, cleavage rate, good quality embryo rate, good quality blastocyst rate and pregnancy rate were compared between the three groups. 2. The 398 treatment cycles with IVF were selected into this study. The sperm acrosome enzymes were measured with the acrosome enzyme kit of shenzhen hua kang company. The 398 cycles were divided into normal group and abnormal group by the results of acrosome enzyme. The outcomes were analyzed in both groups. 3. According to the total PR sperms after preparation, the 642 IVF cycles were divided into two groups: total PR sperm ≥ 1.0×106 group and the total PR sperm <1.0×106 group. The outcomes were analyzed in both groups. 4. The 43 patients with once failed IVF cycle were recruited to be treated with compound Xuan Ju capsule and vitamin C for 3-6 months. The sperm parameters of sperm of the pretherapy and post-treatment were compared in individual.Results:1.There were no significant differences in the teratospermia group and normal sperm morphology group in wife’s average age, BMI, infertility years, proportion of primary infertility, average recovery of oocytes and husband’s average age. The fertilization rate of teratospermia group was statistically significant lower than that of normal sperm morphology group; While there were no significant differences between the two groups in cleavage rate, good quality embryo rate, good quality blastocyst rate and pregnancy rate. Among the three groups by ICSI, there were no statistical differences in wife’s average age, BMI, infertility years, proportion of primary infertility, average recovery of oocytes and husband’s average age. The fertilization rate of severe teratospermia group was significantly lower than that of other groups. But there were no significant differences in other parameters among the three groups. 2. There were no statistical differences of the normal acrosome group and abnormal acrosome group in wife’s average age, BMI, infertility years, proportion of primary infertility, average recovery of oocytes and husband’s average age. The fertilization rate and pregnancy rate from the acrosome enzyme activity of normal group’s were slightly higher than those of the abnormal group, but the differences were not statistically significant. There were no obvious differences in other parameters between the both groups. 3. In the total PR sperm ≥ 1.0×106 group and the total PR sperm <1.0×106 group, there were no statistical differences in wife’s average age, BMI, infertility years, proportion of primary infertility, average recovery of oocyte and husband’s average age. The fertilization rate and good quality embryo rate in the first group were significantly higher than those in the second group. There were no significant differences in other parameters. 4. The sperm parameters, sperm morphology and embryo quality were improved after 3-6 months drug treatment. It can achieve a good pregnancy outcome.Conclusion: 1. The sperm morphology and the total PR sperm after preparation may be important factors affecting the fertilization rate of in vitro fertilization. 2. The sperm acrosome may be little influence on fertilization rate, cleavage rate, good quality embryo rate, good quality blastocyst rate and pregnancy rate. 3. Compound Xuan Ju capsule can obviously improve the sperm parameters, and the quality of embryos and pregnancy outcome.
