Objective:To investigate the effect of two different surgical approaches for small papillary thyroid carcinoma postoperative complications, and postoperative incision length of thyroid hormone supplementation. Methods:Retrospective analysis clinical data 35 cases of unilateral single lobe lesions papillary thyroid microcarcinoma patient be operated in The First Affiliated Hospital of Xinjiang Medical University,Vascular Thyroid Surgery since July 2013 -2014 July.Ultrasound diagnosis before surgery in all cases by the high frequency, And pathologically confirmed diameter<10mm small papillary carcinoma,And all patient are solitary lesion,And for the first time to undergo surgery,Patients had no central lymph node metastasis.Thyroxine preparations after long-term oral treatment of thyroid-stimulating hormone suppression.Divided into two groups according to surgical patients.Group A:Shaver with unilateral gland isthmus resection.Group B:With bilateral lobe resection. For A,B,postoperative complications occurred in two patients, and the prognosis for long-term follow-up.The incidence of using the chi-square test to compare A,B two complications.T-test with incision length and replenishment groups of patients after thyroid hormone. For A,B recurrence rate between the two groups were compared using the chi-square test. Results:A group of 17 cases of postoperative There were two cases of hypocalcemia numbness.Group B 18 cases of postoperative hypocalcemia numbness in 9 cases.A group of 17 cases had postoperative recurrent laryngeal nerve injury, hoarseness were 0 cases.Group B 18 cases of postoperative recurrent laryngeal nerve injury, hoarseness in 1 case.35 patients had no recurrence occurred. The chi-square test showed that the two groups of parathyroid injury incidence appears there is a difference between low calcium numbness, and the difference was statistically significant (P<0.05).However,the rate of recurrent laryngeal nerve injury two sets of data by chi-square test was not statistically significant(P>0.05).The length of the incision and postoperative thyroid hormone supplementation groups were differences in T test analysis, and the difference was statistically significant. Conclusion:The study found differences between the two groups with the extent of surgery postoperative hypocalcemia incidence of complications was statistically significant. The two groups were no cases of recurrence.recurrent laryngeal nerve injury rate of the two groups showed no significant difference.Extent of surgery and recurrence rates of thyroid papillary carcinoma tiny no significant correlation. And the length between the two groups of patients and postoperative incision dosage statistically significant. Thus, the quality of life for patients and reduce postoperative complications beautiful incision, unilateral lobe+isthmus surgical resection more advantages.
