Advances In Surgical Treatment Of Thyroid Microcarcinoma

Thyroid microcarcinoma refers to the tumor maximum diameter≤1cm in thyroid cancer.In recent years,with high frequency ultrasound and pathology diagnosis technology development,the incidence of TMC shows an obvious rising.Due to the high rate of thyroid microcarcinoma, more and more clinician’s pay attention on TMC.While the TMC has small diameter, clinical symptoms are not significant,slow progression, so there is a effect on treatment of TMC and the patients can be long-term survival.However there are different understandings about thyroid microcarcinoma, surgical indications,resection range and central lymph node dissection is on the top of discuss. In this paper we review literatures in the past few years, aged 45 years or less than 15 years old, male, diameter over 5mm, lymph node metastasis,portend a worse prognosis, active surgical treatment should be taken.The operation should be according to clinical characteristics,total thyroid resection,near total thyroid resection,,and also subtotal thyroid resection(ipsilateral thyroid resection,and offside subtotal thyroid resection plus isthmus resection). For the lateral neck lymph node metastasis in patients, possible functional neck lymph node dissection is need. Central lymph node dissection should for the metastasis of central lymph node.But for Central lymph node negative patients, preventive central lymph node dissection is feasible.For patients with thyroid microcarcinoma, how to reduce the risk of tumor recurrence and metastasis, decrease patients with tumor survival rate and improve the life quality of patients,has become a prime target for TMC treatment in patients with follow-up, in which the TMC standard treatment plays a vital role in.
