The Multi-factor Analysis Of Recurrent Time And Types From 151 Gastric Cancer Patients With D2 Radical Gastrectomy

Objective: To analyze the possible factors for recurrent time and types to explore the rule of recurrence and the patients suitable for radiotherapy.Methods: Collecting the medical records of 151 recurrent patients,who had accepted D2 radical operation of gastric cancer,then,treated at Oncology department of the First Affiliated Hospital of Soochow University from August 2012 to October 2013.Summarizing the basic information and the possible factors related with prognosis,including age,sex,primary tumor location,pathological type,operation timem recurrence time,the type of recurrence,the type of Borrmann,TNM classification,treatment,degree of tumor resection,KPS scoring,exploring thr relationship between recurrence and other factors,such as primary tumor location,TNM classification and so on. The relationship between DFS and other relative factors is analyzed by Kaplan-Meier,the dependent effecting factors of DFS about the patients who accepted D2 radical operation are analyzed by COX test.The relationship between recurrent types and other factors are analyzed by X2 test,it is regarded statistical meaning,if the result shows P<0.05.Results: The average DFS of the patients accepted D2 radical operation is 575 days,The median DFS is 414 days.Single factor analysis showed that age,T classification,N classification,Borrmann types are obvious related with DFS of the gastric cancer patients accepted D2 radical operation;Sex,primary tumor location have no apparent raltionship with DFS.COX test showed the T classification(P<0.01)and N classification(P<0.01)are the dependent effecting factors for DFS.Groping statistics showed that the locoregional reccurence rate of the patients belong to N1 classification is 59%,the patients belong to N2 classification is 47%.The locoregional recurrence is the major style for N1 N2 classification.The X2 showed that T classification have no apparent relationship with the recurrent types(P=0.263),N classification have apparent relationship with the recurrent types(P=0.010),the primary tumor location including Cardia-Fundus ventriculi,Corpora venticuli,Sinuses ventriculi have apparent relationship with the recurrent types(P=0.015).Conclusions:The average DFS is 575 days.Age,T classification,N classification,Borrmann types are the effecting factors for DFS.T N classification are the dependent effecting factors for DFS.They are protective factors if the T N classification are on the earlier stage.The primary tumor location is related with the recurrent types in statistics area.The rate of the locoregional recurrence is higher than the primary tumor location is at Sinuses ventriculi.N classification have apparent relationship with the recurrent patterns,The locoregional recurrence is the major style for N1 N2 classification.The later N classification,the more distant metastases.The patients may benefit more from radiotherapy if they belong to N1 N2 classification and the primary tumor location is at Sinuses ventriculi.
