Objective: To investigate clinical application of palatal roll connective tissue flap in the esthetic zone implantation.Methods:. 26 cases with the labial depressed soft tissue in esthetic zone two staged implantation. “H” incisions were operated on top of the alveolar ridge, the two vertical incisions were deep to mucoperiosteal and the horizontal incisions were under the mucosa, the palatal side mucosa was separated from the lower connective tissue. Labial mucoperiosteal pedicle flap tipped with a palatal flap of connective tissue flap was formed. Then palatal connective tissue was folded and inserted under the labial gingival flap. Papilla around the implant was shaped with healing abutment. The crowns were loaded after 2-4 weeks and the labial soft-tissue change was measured after 6 months.Results: All the implants were stable, and the improvement of labial soft tissue fullness was 1.10±0.45 mm with symmetrical gingival margin and satisfied outline. The distance of papillae top to crown interproximal contact point was 2.05 ± 0.76 mm.Conclusions: The palatal roll connective tissue flap is easy survival and useful in the esthetic zone implantation.
