ObjectiveThrough the applicative research of test Anti-mitochondrial antibodies in the early diagnose primary biliary cirrhosis(PBC),in order to early diagnose and treat primary biliary cirrhosis,delay the disease progress and hepatic sclerosis, improve the patient’s quality of lifeThis paper will also explore joint inspection with other autoantibodies and AMA,to find the significance of early diagnosis of PBC and improve the early diagnostic sensitivity and specificity of PBC.Methods260 cases of the subject into two groups: the experimental group and the control group. Through indirect immunofluorescence and Western blot to test ANA,AMA,ds-DNA, anti-Ro52 antibody,AMA-M2, M2-3E antibody, anti-Sp100 antibody, anti-PML antibody, anti-Gp210 antibody, anti-LKM-1 antibody, anti-SLA/LP antibody, anti-LC-1 antibody, anti-n Rnp antibody, anti-Sm antibody, anti-SSA antibody, anti-SSB antibody, anti-Scl-70 antibody, anti-PM-Scl antibody, anti-Jo-1 antibody, Anticentromere antibodies, anti-PCNA antibody, Anti-nucleosome antibody, anti-histone antibody, Antiribosomal P protein antibodiesResultsIn the ANA detection, Cytoplasm particle type positive rate of the experimental group is higher than the control group(P=0.021),differences are statistically significant.Other karyotypes’ positive rate in the experimental group and control group have no difference(P>0.05). In the AMA detection, AMA positive rate of the experimental group is higher than the control group(P=0.047), differences are statistically significant.AMA-M2 positive rate of the experimental group is higher than the control group(P=0.007), differences are statistically significant. M2-3E antibody positive rate of the experimental group is higher than the control group(P=0.047), differences are statistically significant. In other autoantibodies detection, only anti-Gp210 antibody positive rate of the experimental group is higher than the control group(P=0.045),differences are statistically significant. In the result of compare the experiment indexes’ sensitivity and specificity, we find Cytoplasm particle type’s sensitivity of early diagnostic PBC is 71.43%, specificity of early diagnostic PBC is 96.99%. Sensitivity of use AMA-M2 to early diagnostic PBC is 100%, specificity of use AMA-M2 to early diagnostic PBC is 96.99%. Sensitivity of Sensitivity of use M2-3E to early diagnostic PBC is 85.71%, specificity of use AMA-M2 to early diagnostic PBC is 98.50%.Anti-Gp210 antibody’s sensitivity of early diagnostic PBC is 42.86%, specificity of early diagnostic PBC is 98.50%. According to the Receiver Operating characteristic Curve with Cytoplasm particle type, AUC(aere under the ROC curve)=0.842,SE(standard error)=0.103, P=0.002; According to the Receiver Operating characteristic Curve with anti-Gp210 antibody, AUC=0.778,SE=0.118, P=0.013. ROC curve area is greater than 0.7, the detection of ANA cytoplasm particle type and anti-Gp210 antibody for diagnosis of PBC with moderate accuracy. According to the Receiver Operating characteristic Curve with AMA-M2, AUC=0.985,SE=0.009, P<0.001; According to the Receiver Operating characteristic Curve with M2-3E, AUC=0.921,SE=0.078, P<0.001. ROC curve area is greater than 0.9, the detection of AMA-M2 and M2-3E for higher accuracy in the diagnosis of PBCConclusion1. AMA subtypes detection have clear significance for early diagnosis of PBC。AMA-M2 test joint inspection with M2-3E test, can improve sensitivity and specificity to 100% and 98.5%.2. In other autoantibodies test of early diagnose PBC, Cytoplasm particle type and anti-Gp210 antibody have clear significance. Anti-Gp210 antibody’s specificity is 98.50%,is higher than other indexs. These two indexs can be used in diagnosis of PBC as the basis of AMA negative PBC patients.3. Anti-Ro52 antibody, Anticentromere antibodies, anti-Sp100 antibody, anti-PML antibody and anti-SLA/LP antibody have no statistical significance for early diagnosis of PBC.Anti-LKM-1 antibody, ds-DNA,anti-LC-1 antibody, anti-n Rnpantibody, anti-Sm antibody, anti-SSA antibody, anti-SSB antibody, anti-Scl-70 antibody, anti-PM-Scl antibody, anti-Jo-1 antibody, anti-PCNA antibody, Anti-nucleosome antibody, anti-histone antibody and Antiribosomal P protein antibodies have clear meaning for early diagnosis of PBC。...
