Ilizarov Skill To Treat Tibial Traumatic Osteomyelitis Of Cierny-Mader Type Ⅳ

Objective:To discuss clinical outcomes of Ilizarov technique in the treatment of tibial traumatic osteomyelitis of Ciemy-Mader types Ⅳ.Methods:From april 2012 to december 2014 we treated 14 patients with tibial traumatic osteomyelitis of Cierny-Mader type Ⅳ. There were 10 men and 4 women,aged from 13 to 64 (average,34.5 years).Early debridement and amputated necrotic bone lesions,install the ring external fixantion in metaphyseal osteotomy minimally invasieve wound lavage with closed suction drainage or open VSD dressing or biological material covered two take the skin graft.Application Ilizarov bone segment extending technology to repair bone defects.Regular follow-up, according to the healing of bone X-ray shown in the gradual dismantling of the fixed components.Results:14 cases were followed up for 12 to 26 months, with an average follow-up of 16.4 months. Healed without recurrence of infection with varying degrees of bone exposed personalized treatment.Amputated after the lesion sequestrum bone defects 3.5～7.0 cm, average 5.2 cm, the mean bone handling time 11.6d / cm, are to achieve bone healing and restore normal length.Three cases occurred slip bone segment axially offset by the extension osteotomy adjustment module back to normal power lines; according to the rate of fracture healing Palay evaluation criteria, the results: excellent in 8 cases, good in 4 cases,2 cases, good rate of 85.7%.Conclusion:traumatic tibial osteomyelitis Cierny-mader divided according to type 4, however, the ring Ilizarov external fixator treatment Cierny-mader Ⅳ traumatic distal tibial osteomyelitis effective therapy.
