Objective:To compare the renal protective effect of Ulinastatin and mannitol on children with congenital heart disease whom renal injury during cardiopulmonary bypass (CPB), to provide further theoretical basis for clinical therapy. Methods:retrospective analysis of hospitalized in fifth affiliated hospital of xinjiang medical university between March 2013 and august 2014, underwent open heart surgery under cardiopulmonary bypass (CPB) in children with congenital heart disease. Our patient cohort is divided ink. those renal protected with Ulinastatin during intraoperative and postoperative 3 days(Ulinastatin group,n=46) and those renal protected with mannitol during intraoperative and postoperative 3 days(mannito group,n=36),the concentration of serum creatinine(Cr),urea nitrogen(BUN)and urinary volume during intraoperative and postoperative l,3days.Results:The patients in two groups were successfully completed operation, all patients were come to diagnostic agreement. There was no significant difference between Ulinastatin group and mannito group in preoperative serum creatinine, urea nitrogen, age, sex, complicated degree of congenital heart diseas operation time, CPB time, amount of bleeding. The BUN and Cr level in first day after operation was significantly higher than preoperation evel(P>0.05). Compare the level of Cr and BUN after first day and third day, there was statistical significance between two groups(P< 0.05).Conclusion:Both methods play good role in renal protection, but Ulinastatin certain and obveos effect on renal protenction and gel favorable result than mannito effect on renal protenction.
