Objective:To assess the iodine concentration of hepatic alveolar echinococco-sis (HAE) using spectral computed tomography (CT) with comparison of [~18F]flu-o rodeoxyglucose positron-emission tomography (FDG-PET), and to explore the val-u e of spectral CT for evaluation of HAE viability. Methods:18 patients with his-t ologically confirmed or clinically proved HAE underwent spectral CT and FDG- P ET examinations. After three-phase (arterial phase (AP), portal venous phase (P-V P) and phase (VP)) scan, the raw data transfer to advanced 4.6version work stati oin for measurement. The quantitative iodine-based material decomposition im-ages and optimal monochromatic image of spectral CT were reconstructed and io-dine concentration (IC) was measured in different organizational structures (hydati-d tis sues, edge tissue of the lesion, marginal zone of lesion and normal liver). Io-dine results of different phases and different tissues were compared with Rank test. T he correlation of iodine value and PET suv were performed with Sperarman m-eth od. Results:FDG-PET identified increased metabolic activity in the correspond-ing lesions in 13 patients (13/18,72.2%). The iodine concentration in marginal z-one of lesion were significantly higher than in solid component of lesion and nor-mal liver parenchyma during PVP and VP. Positive FDG-PET findings with "hot pot" were also positive at spectral CT images which displayed as continuous or disco ntinuous ring-like enhancement with the 5~10mm thickness. The iodine valu-e of hydatid tissues showed no statistically significant difference (P>0.05). The i-odin e value of edge tissue of the lesion and normal liver and iodine value of no-rmal liver tissues showed statistically significant difference (P<0.001). There was corr elation between IC and SUV in marginal zone of HAE lesion, and it was hi-ghes t during PVP (r=0.873, P<0.001) There was low between CT values and suv. C onclusion:There were good correlation between spectral CT and FDG-PTE. S-pec tral CT could be recommended as a more practical tool in the clinical routine.
