Study Of Safety And Effects Of Risperidone, Olanzapine Combined With Trazodone On Cognitive Function In Schizophrenia Patients

Objective: To evaluate the efficacy and safety of risperidone, olanzapine combined with trazodone on cognitive function in schizophrenia patients.Methods: A total of one hundred sixty patients with schizophrenia were randomly divided into four groups: risperidone, olanzapine, risperidone combing trazodone and olanzapine combing trazodone. The dosage of risperidone was 1mg/day in the beginning, then added to 4-6mg/day according the effects; the dosage of olanzapine was 5mg/day in the beginning, then added to 10-20mg/day according the effects; the dosage of trazodone was 50-100mg/day. Before and after 6 weeks treatment, clinical efficacy was evaluated by Positive and Negative Syndrome Scal(PANSS), cognitive function was assessed by Wisconsin Card Sorting Test(WCST), MATRICS Consensus Cognitive in Schizophrenia(MCCB) and Personal and Social Performance scale(PSP). Treatment Emergent Symptom Scale(TESS) was assessed during treatment. Using SPSS13.0 to analysis the database.Results:(1) There was no significant different of current age, onset age, age of first hospitalization, level of education, the scores of PANSS and cognitive test before treatment during groups. And the scores of PANSS were decreased significantly after treatment 6 weeks, there were statistically significant(P<0.05), but there were no difference between the four groups at the same time.(2)By comparing those groups’ MCCB at different times in the same group or at the same time in different groups before and after treatment, the scores of CPT-IP, SC, HVLT-R, CF in those groups were all significantly higher than those before treatment(P<0.05). And the risperidone combing trazodone group, the olanzapine combing trazodone group were higher than the group of risperidone and olanzapine(P<0.05), but between groups, there was no statistically significant(P>0.05).(3)By comparing those groups’ WCST at different times in the same group or at the same time in different groups before and after treatment, the scores of Cc, Re, Rp, n Rpe in those groups were all significantly higher than those before treatment(P<0.05). And the combing groups were higher than the group of risperidone and olanzapine(P<0.05), but there was no statistically significant(P>0.05).(4) By comparing those groups’ PSP at different times in the same group or at the same time in different groups before and after treatment, the scores of four groups significantly increased after treatment, and the jointtreatment groups were improved more significantly(P<0.05).(5)During the treatment, each group had adverse reactions, and the olanzapine combing trazodone group waslower than other groups, and the adverse reactions were mild, patients could tolerate more adverse reactions.(6)Correlation analysis showed: the scores of negative symptoms and the level of education were significantly related to HVLT-R, SC, ME, DS, Rp and n Rpe(P<0.01). And the main factors influencing cognitive function of schizophrenia were the level of education and the scores of negative symptoms.Conclusion:(1) Risperidone and olanzapine combing trazodone or Risperidone and olanzapine can improve the mental symptoms and cognitive functon in schizophrenia patients.(2) Risperidone and olanzapine combing trazodone could obviously decrease the adverse reactions incidence, patients could tolerate more adverse reactions and increase the patient’s compliance.(3) The scores of negative symptoms and the level of education were significantly related to cognitive function of schizophrenia patients.
