Background:Burden of disease means the loss and impact on health, society and economics caused by disease, disability and premature death, including epidemiological burden and economic burden. At present, cancer is one of the most serious burden of disease among all chronic non-communicable diseases. China is currently the world’s largest developing country, the death caused by cancer is the primacy of the national cause of death, of which the mortality and incidence of lung cancer is in the first place. At the macro level, by affecting the productivity of labor and social development and increasing health system expenditure, cancer will reduce the level of macro-economic development. At the micro-economic level, cancer will cause the loss of family labor and income, leading to impoverished families finally. Due to economic and social development, medical technology advances, changes in the age structure of the population and income increases, and many other factors, the rapid growth of health care costs, medical expenses rises far more rapid than the rate of increase of the GDP and price index. Hospitalization expenditure is the main part of medical expenses and it reflects not only the utilization of health resources but also the direct economic burden of patients,and experts and scholars at home and abroad have conducted in-depth study of it. Controlling hospital costs is not only important for Chinese government and patients, but also beneficial to ease the tension between doctors and patients in our country.Objective : A retrospective research method was used to study the hospitalization expenditure and its influencing factors among lung cancer patients in Jiangsu Cancer Hospital from 2005 to 2011, and the major objective was to find the key factors. This article will also provide the policy suggestion and reference for the establishment and improvement of single disease management and control system.Method:We selected Jiangsu Cancer Hospital for each of the discharge of hospital fees and medical records diagnosis of lung cancer from HIS, excerpted the medical record information data in need, Excels were used to set up the databases and SPSS 20.0 package was used to do descriptive analysis, rank sum test and multiple stepwise regression to all the cases selected.Result:(1) The lung cancer inpatients has increased every year, the highest chain growth rate has raised up to 29.8% during seven years, and female lung cancer incidence was significantly.(2) The average hospitalization expenditure of lung cancer patients is up to 20172.13 yuan, it leaves a heavy economic burden of disease.(3) The daily hospitalization expenditure has increased year by year, the 7-year cumulative growth has reached 73.6%.(4) The structure of hospitalization expenditure is not reasonable, the nursing care and treatment costs take low proportion, so it is difficult to value the service and technique of medical works. Medical fee, the average proportion as highly as 64.8%, takes the biggest proportion of the hospitalization expenditure.(5) The analysis of single factor reveals that age,hospital stay, drug proportion, year of discharge, job, residence, surgery and payment method all influenced the hospitalization expenditure.(6) Multiple regression analysis reveals that drug proportion and surgery has the greatest impact on the hospitalization expenditure.Conclusion:(1) The incidence and mortality of lung cancer in our country is so high that the epidemiology burden is heavy, we should actively take measures to protect the environment. Implementation of tertiary prevention is necessary to reduce the incidence of lung cancer.(2) The direct financial burden of lung cancer is too heavy for patients to bear, and it is feasible for our government to improve the critical diseases compensation ratio, and improve the medical insurance compensation scheme.(3) The daily hospitalization expenditure has increased year by year, health management should be strengthened, and ensure that the therapeutic effect while reducing the average length of stay.(4) Drug control is the key to reduce the proportion of direct economic burden of patients. Aiming to reduce the drug proportion effectively, medical workers should regulate their clinical conducts, and our government is expected to establish an effective compensation mechanism.
