Study Of TCM Types Distribution And Relativity With Sex Hormones In Low Ovarian Function

[Objective]The low ovarian function disease is including contains Decreased Ovarian Reserve and Premature Ovarian Failure, domestic incidence increased year by year, harm many women’s physical and mental health and quality of life, left Ovarian Reserve function refers to the ovaries can raise the number of follicles and quality ofresponse the female’s reproductive potential and reproductive endocrine function. Ovaries to produce fewer oocytes, egg quality decline, resulting in a decline in fertility and the lack of sex hormones, called the ovarian reserve function by [107].Incidence of ovarian reserve function decline in the general population was ten percent[108], after investigation in 1-6 years for the development of ovarian premature senility[1]. Etiology is not clear at present, the western medicine mainly taking hormone replacement therapy, long-term use increase the risk of endometrial cancer, breast cancer, become one of the clinical refractory disease of departmentof gynaecology. This study was investigating the distribution for TCM type of DOR and POF and sexual hormone FSH, LH, E2. Make the observation for TCM treatment and explore the mechanism of treatment of traditional Chinese medicine and clinical experience of the mentor.[Methods]Part One:Selected 118 cases were form July 2004 to March 2015 China Academy of Chinese Medical Science XIYUAN Hospital. Gather statistics and analysis for the low ovarian function disease’s TCM types’distribution and relativity with sex hormones, such as E2, bFSH, bLH. Part Two:Selected 31cases were form July 2014 to March 2015 China Academy of Chinese Medical Science XIYUAN Hospital. Total 31 cases were the low ovarian function disease in liver and kidney yin deficiency. All patients were treated with nourishing liver and kidney essence and regulating Chong and Ren.[Results]Part One:1. The main TCM type for the low ovarian function disease is liver and kidney yin deficiency type. ovarian dysfunction syndrome differentiation frequency list:Yin deficiency of liver and kidney in 57 cases (48.31%), Yang deficiency of spleen and kidney in 38 cases (32.20%),16 cases of kidney deficiency and liver stagnation (13.56%),4 cases of kidney deficiency and blood stasis (3.39%), the other 3 cases (2.54%). Among the 3 cases of other syndromes including kidney blood loss in 2 cases (1.69%),1 cases of deficiency of Qi and blood (0.85%) two.2. Analysis of each TCM types for sex hormones level in pre-treatment:E2 level of liver and kidney yin deficiency type is considerably lower than other types, such as spleen-kidney yang deficiency, liver depression and kidney deficiency, deficiency of kidney and blood stasis.(P<0.01)3. Compared non-liver and kidney yin deficiency type patients, E2 level before treatment difference was very significant (P< 0.01), with statistical significance.Part Two:1. In the integarated effect:Treat 31 cases of DOR and POF three menstrual period,6 cured (cure rate 19.35%),1 (effective rate was 3.23%),19 (61.29% efficiency), invalid 5 (no 16.13%), the total effective rate of 83.87.%.2. Changes of sex hormones:E2 level was improved after treatment and before treatment, the difference was very significant (P<0.01), with statistical significance; the FSH/LH level after treatment than before treatment decreased slightly, the difference was not statistically significant (P>0.05); the FSH level after treatment than before treatment decreased, the difference was statistically significant (P<0.05); The LH level after treatment than before treatment, the difference was statistically significant (P<0.05).3. Change of TCM syndrome Compared with pre-treatment, after treatment the score of patient menstruation decreased considerably.(P<0.5)4. changes of menstrual status:31 patients in the treatment of traditional Chinese medicine after the menstrual symptom score decreased, the difference was statistically significant improvement (P<0.05).[Conclusion]1. Ovarian dysfunction in the liver and kidney yin deficiency syndrome is the main syndrome type of patients before treatment, the level of E2 was significantly decreased.2. TCM with nourishing liver and kidney, regulating Chong and Ren method was adopted in the treatment of ovarian dysfunction of liver and kidney yin deficiency, after treatment E2 was significantly increased, and the levelof FSH、LH is without statistical significance.3. TCM with nourishing liver and kidney, regulating Chong and Ren method was adopted for the disease of liver kidney yin deficiency, improve menstruation, menstruation, amenorrhea, the ovarian function and fertility can be gradually returned to no rmal, but also relieve hot flashes sweating, vaginal dryness, dry mouth and throat, insomnia, loss of libido, Yaoxisuanruan low estrogen level. The symptoms are reduced so the quality improved of life of patients.
