Background:Intensive care unit (ICU) is the central department for managing critically ill patients and some high-risk patients after operation. The medical and nursing task of ICU are painstaking and arduous. In the current situation of medical and nursing shortage, how to effectively calculate the workload and improve work efficiency is the research focus. In our country, the most commonly used method of nursing manpower deployment is grading nursing method, which lacks objectivity, flexibility and accuracy. The TISS scoring system is positively correlated with the direct nursing time and could accuratly evaluate the workload of nursing staff. Based on this, we carried out the following research.Objective:In this study, we aimed to explore the more effetive method for nursing workload evaluation, we used double TISS scoring system to evaluate the nursing workload for each nurse in cardiac sugery ICU of Qilu hospital of Shandong University. We also used questionnaire to collect their recommendations for the nursing workload evaluation and TISS scoring system.Methods:Through investigation and questionnaire, we counted the number of nurses who left or applied to leave Cadiac Surgery ICU between 2011 and 2013. We evaluated their satisfaction degree for the current workload evaluation and found related problems. In March,2014, we used both of TISS scoring system and current workload evaluation to calculate the workload of each nurse in our unit, and finally made a comparison of the two methods.Results:Before the implementation of TISS system, large amounts of nursing staff are not satisfied with the current workload evaluation, and nearly 20% of them to apply to leave ICU. We found that the main related problems causing the dissatisfaction with the current workload evaluation and were fairness and accuracy. After the using of TISS scoring system, the nursing staff satisfaction degree significantly elevated, and the nurse number of applying for leaving ICU remarkablely dropped. The workload statistics of Cardiac Surgery ICU nurses in March,2014 displayed that TISS scoring system could more accuately calculate the workload of each nursing staff. Compared with current workload evaluation, TISS scoring system showed that high qualification nurse who cared more critically ill patients had more workload, so as to their workload of day shift, swing shift and graveyard shift.Conclusion:TISS scoring system could more effectivly calculate the workload of ICU nursing staff, thereby improve their job satisfaction and the nursing quality. By calculating different shifts workload with TISS scoring system, the Nursing administrators could make reasonable arrangements of nursing work and reduce medical accidents.
