| Health information sharing improves the performance of patient self-management when dealing with challenging chronic disease care. Cloud computing has the potential to provide a more imaginative long-term solution compared with traditional systems. However, there is a need for identifying a suitable way to share patient health information via cloud. This study aims to identify a promising and achievable cloud based solution which enables privacy protected sharing when multiple stakeholders are involved for different purposes.This paper starts with analyzing the domestic and overseas state of art in this area based on the facing problems, put efforts on the studying of Health information sharing solution in Sweden. And then proposed a hybrid cloud based patient centered platform for sharing health information. The main works of this paper are as follows:1. Based on the analysis and comparison of health information sharing, chronic disease self-management and the four cloud deployment models, this paper proposed to deploy the health information sharing platform by hybrid cloud deployment model. And all the related contents were classified into roles, objects, health records and procedures by modeling the whole platform. Implemented the hybrid cloud platform by deploying with OpenStack and Amazon Web Services, and implemented the OpenStack Swift encryption module as a Swift middleware.2. Proposed the sectionalized electronic health record based on the traditional health information sharing methods, and integrated with hybrid cloud. It gives patients the rights to their own data, and solves the problem of privacy protecting when sharing sensitive health information among trusted and semi-trusted stakeholders. And implemented a function module for supporting sectionalized electronic health record with open source lib dom4j.3. Designed and implemented the client of the prototype system, it includes utilizing Joss API and AWS SDK for connecting with cloud side, implementing the updated health record noticing method based on Feed subscription method, and the client GUI interface by Swing.The research results were tested and evaluated by electronic health record sharing testing, Swift uploading and encrypting testing, the area expert interviews and the qualitative research verification. The proposed solution obtained positive evaluation feedback. The vertical prototype system realized the recording of health information and the privacy protected sharing with hybrid cloud, achieved the objectives of chronic disease patient self-management and to promote the long-term monitoring and treatment of chronic disease. The cloud based health information sharing will highly improve the utilization of information resource, and will probably change the present health service model which was facing serious challenges. |
