Observing The Recent Clinical Efficacy Of The Medicinal Broth By The Composition Of THH And Leonurusin In Combined Treatment Of Lupus Nephritis

ObjectiveTo assess the efficacy and safety of the medicinal broth by the composition of Tripterygium Hypoglaucum Hutch (THH) and Leonurus in the treatment with Lupus Nephritis (LN).At the same time, tryed to further evaluate the role of the urine biological markers including NGAL and TGF-β1 in the onset process with LN, which will be hoped to provide a meaningful reference on clinical work of LN.MethodsIn this study, the 32 patients with LN of mild-to-moderate activity as the research objects, which come from the department of rheumatology outpatient and inpatient of the first affiliated hospital of guangzhou university of TCM in March 2014 to March 2015. According to the different treatment measures, they are divided into two groups, experimental group with 22 cases,10 cases of control group. All patients included in the study have taken the foundation treatment of Western medicine. In addition, the experimental group takes the medicinal broth (THH and Leonurus), while the control group uses cyclophosphamide (CTX), three period of treatment, each course of 4 weeks, a total of 12 weeks. The process needs to observe and evaluate some informations, including TCM Symptoms Scale, SLEDAI, BILAG with kidney system, relevant laboratory indexes and adverse events, and collect urine samples from all of the patients which will be stored in low temperature with -70℃, and then tested for urine NGAL, TGF-β1 by ELISA kits. All the above will be used SPSS18.0 statistical software for statistical analysis.Results1、The two groups after treatment can effectively improve the SLEDAI score and BILAG score with kidney system, rise complement, lower levels of anti-dsDNA, erythrocyte sedimentation rate (ESR) and urinary protein, but the effect of experimental group is better, and also improve TCM symptom scores. The treatment of two group has not influence on the white blood cells, liver function and serum creatinine obriously, while can reduce triglyceride levels. Menstruation is abnormal, the experimental group in 1 case, the control group in 3 cases. There are 4 cases of control group of digestive discomfort. In addition, the control group appear chicken pox in 1 case,3 cases of the bronchitis of recurrent.2、Urine NGAL and TGF-β1 of the patients with LN which contain higher levels can be reduced obviously after treating 8 weeks. Furthermore, the experimental group has the obvious effect in terms of reducing urine NGAL by the statistical tests.Conclusion1、The medicinal broth has certain effect in controlling inflammation, reducing the urine protein and remissing the disease, especially in improving the clinical symptoms and complement 3(C3), reducing the incidence of the infection and menstrual abnormalities, reducing the urine protein quickly. What is more, it is safe and convenient as be boiled only at home, which can promote the quality of life and reduce the psychological burden for the patients. For this reason, it is another good way selecting the relatively mild but effective drug such as the medicinal broth for the patients with LN who is not suitable or reluctant for using CTX.2、Urine NGAL and TGF-β 1 have important value in terms of assessing the fibrosis and acute injuries of kidney. The medicinal broth by the composition of THH and Leonurus plays a role in the treatment with LN, which may be related to reducing the levels of urine NGAL and TGF-β1.
