ObjectivesPrimary medical staff are an important part of primary health care institutions; job satisfaction, initiative and job burnout directly affect the enthusiasm and creativity of their work, and directly determine the quality and efficient of basic public health services provided by primary health care agencies. Based on self-administered questionnaires completed by primary health care providers in Gansu Province, job satisfaction and job burnout, this study seeks to understand the status of primary health service institutions medical staff; to find the main factors affecting primary medical staff job satisfaction and job burnout; to analyze the relationship between the various dimensions of job satisfaction as well as overall satisfaction and burnout; to put forward targeted countermeasures and suggestions in order to improve job satisfaction and to reduce job burnout and to promote the improvement of primary health institutions three-level health service network in Gansu Province; and finally, to provide safe, reliable, high-quality, low-cost and convenient services for the urban and rural residents of Gansu Province.MethodsOn-site self-administered questionnaires were used for the field investigations and subjected to quality control. Survey data were statistically analyzed using SPSS 20.0, using independent samples t-test and ANOVA to explore statistical differences in various dimensions of job satisfaction and overall satisfaction by different demographic factors among medical staff; multivariate linear regression analysis was used to identify the main factors affecting job satisfaction; correlation analysis was used to analyze the association between job satisfaction and job burnout.Results1. The basic characteristics of respondents:Female medical personnel accounted for 67.4%of total respondents.29 years old and under comprised the majority of the age distribution, accounting for 51.5% of total respondents.69.7% were married; 61.0%had a vocational school degree.49.5% had the occupational title of junior staff. 46.6%of the medical staff had 5 years or fewer of working experience and 2.8% had more than 30 years of working experience. Doctors accounted for 30.3%, nurses for 22.3%, logistics personnel for 3.5%; 71.7% were permanent staff.2. Job satisfaction and job burnout:Out of five dimensions of job satisfaction, work reward satisfaction was the lowest, with a mean score of 2.60 points, indicating a low level of satisfaction. This was followed by hospital management satisfaction and occupational social environment satisfaction, with mean scores of 2.98 and 2.99, respectively, also suggesting low levels of satisfaction. Direct job satisfaction and internal workplace environment satisfaction reached medium levels, with mean scores of 3.19 and 3.37, respectively. The mean job burnout score was 2.80.3. Analysis of job satisfaction and job burnout:A high degree of professional risk, occupational pressure and stress, and occupational challenges were the main factors influencing the direct job satisfaction. Lack of personnel and appropriate equipment were the main factors influencing the internal workplace environment satisfaction. Low unit pay and compensation for effort were the main factors affecting work reward satisfaction. Degree of satisfaction on each item under hospital management were below average; problems with hospital management systems, promotion system and democratic management need to be addressed. Low social status had a great negative effect on occupational social environment satisfaction. Apathy for patients and stress were the main risk factors for job burnout.4. Analysis of job satisfaction and job burnout influence factors:(1) Satisfaction and burnout were not significantly different by sex (p> 0.05); hospital management satisfaction and occupational social environment satisfaction were significantly different by marital status (p< 0.05), but it did not significantly affect the work itself, internal workplace environment,work reward,overall job satisfaction or burnout (p> 0.05); age was significantly associated with direct job satisfaction and work reward satisfaction (p< 0.05),but not associated with the internal workplace environment, hospital management satisfaction, occupational social environment,overall job satisfaction or burnout;different eduacational levels did not affect thedirect job satisfaction, overall job satisfaction or burnout,but it did affects the returns on job satisfaction, hospital management satisfaction, work environment satisfaction and overall satisfaction(p<0.05); Different titles on the job satisfaction internal environment, hospital management satisfaction, social practice environment satisfaction and job burnout difference had no statistical significance, in return for the work itself satisfaction, job satisfaction and job overall satisfaction difference was statistically significant (p< 0.05); Different working fixed number of year of the medical staff work itself satisfaction and job reward satisfaction difference was statistically significant (p< 0.05), other dimensions of satisfaction, no statistically significant difference were observed in the overall satisfaction and job burnout; Different health professionals on the various dimensions of satisfaction, total satisfaction and job burnout difference had statistical significance (p< 0.05); Internal environment, different forms of employment to the work satisfaction and the job practice environment satisfaction difference was statistically significant (p< 0.05), other dimensions of satisfaction, there was no statistically significant difference total satisfaction and job burnout.(2) The regression analysis results show that junior title and no title, direct job satisfaction, internal workplace environment, work reward satisfaction, and hospital management satisfaction have significant effects on overall job satisfaction;(3) The correlation analysis results show that there exist statistically significant negative correlations between job burnout and job satisfaction as well as its five dimension satisfaction (P< 0.01), with a large Pearson correlation coefficient between direct work satisfaction and overall job satisfaction.Conclusions(1) The average score of job satisfaction among primary health service providers in Gansu province is 3.01, and the average score of job burnout is 2.80, indicating that overall job satisfaction and job burnout among primary medical staff is generally low.(2) Scores on the five dimensions of job satisfaction, from high to low, were internal workplace environment (3.37), direct work satisfaction (3.19), occupational social environment satisfaction (2.99), hospital management satisfaction (2.98), and work reward satisfaction (2.60).(3) In the study of different demographic segments, job satisfaction and job burnout was statistically different according to degrees, occupational titles, and positions (p< 0.05); the other demographic characteristics were not significantly associated with job satisfaction and job. At the same time, demographics were significantly associated with individual satisfaction dimensions.(4) Through multiple linear regression analysis with a total of six factors identified hospital management satisfaction, work reward satisfaction, internal workplace environment satisfaction, direct job satisfaction, no title and primary title, as the the influence factors from large to small effect size.(5) The correlation analysis results show that a negative correlation exists between the overall job satisfaction as well as each dimension of job satisfaction and job burnout among medical staff in Gansu Province.
