Objective To observe the ventilation effect of I-gel laryngeal mask used for endoscopic thyroid surgery patient in general anesthesia.Methods Select 60 cases of endoscopic thyroid surgery patient in general anesthesia, which are randomly divided into 3 groups(n=20): the group of patients with endotracheal intubation(Group Q), the group of patients with Supreme laryngeal mask(Group S) and the group of patients with I-gel laryngeal mask(Group I). Record success rate of insertion, duration of insertion, oropharyngeal leak pressure(OLP)of Group S and Group I, and score with fiberoptic bronchoscopy(FOB)to evaluate the use of laryngeal mask. Record heart rate(HR)、mean arterial pressure(MAP)、pulse oxygen saturation(Sp O2) at such points as before insertion(T0), 1 min after insertion(T1), 3 min after insertion(T2), before the establishment of CO2 operating space(T3), 30 min after the establishment of CO2 operating space(T4), 10 min after the resection of specimens(T5), 1 min before removal of laryngeal mask(T6) and 1 min after the removal of laryngeal mask(T7), and record end-tidal carbon dioxide partial pressure(PETCO2)and peak airway pressure(Ppeak) form T1 to T5. Record after the removal of laryngeal mask the occurrence of reflux and aspiration, voice hoarse,sore throat and other adverse reactions.Results It is not statistically significant about whether the insertion is successful or not. Compared with Group Q, the time consumed in insertion in Group I and S is significantly reduced(P<0.05). The differences in Group S and I about OLP and FOB score are not statistically significant. Compared with Group Q, the rise of MAP and HR in T1, T6, and T7 in Group S and I is significantly lower(P<0.05). The comparison of differences in Sp O2,PETCO2 and Ppeak in the three groups is not statistically significant. Compared with Group S and Q, in Group I the occurrence of sore throat and painful swallowing is significantly lower(P <0.05).Compared with Group S, in Group I the occurrence of laryngeal mask body with blood is significantly lower(P <0.05). Reflux,aspiration and voice hoarse in the three groups is not statistically significant.Conclusions Both I-gel laryngeal mask and Supreme laryngeal mask can be used safely and effectively in airway management of endoscopic thyroid surgery patients, and the I-gel laryngeal mask will be less hurting and more comfortable.
