Background Stroke has an increasing trends in the IBD patients, but the issue about whether IBD is an independent risk factor of stroke is controversial. Objectives To systematically review the risk of stroke in the IBD patients. Methods We electronically searched Pubmed. Embase、CNKI、VIP、 Wanfang database and manually searched some related Chinese journals.Meta-analysis was conducted using Stata12.0 software after cohort study and case-control study revealing the relation between IBD and stroke were filtered and included. Results A total of six cohort study were included and meta-analysis show that the risk of stroke in IBD patients have statistical significance (RR=1.15, 95%CI:1.08-1.24) comparing with general populations.Sub-group analysis show that when comparing with general populations, the risk of stroke in both CD and UC patients have statistical significance[CD:(RR=1.21,95%CI: 1.08-1.36); UC:(RR=1.13,95%CI:1.05-1.21)];the risk of stroke in IBD patients of both Taiwan and European and American areas have statistical significance [European and American area:(RR=1.16.95%CI:1.07-1.26); Taiwan area:(RR=1.12,95%CI:1.01-1.23)];the risk of stroke in elderly IBD patients have statistical significance (RR=1.13,95%CI:1.03-1.23),while the young’s doesn’t (RR=1.10,95%CI:1.00-1.20).the risk of stroke in female IBD patients have statistical significance (RR=1.13,95%CI:1.01-1.26),while the male’s doesn’t (RR=1.12,95%CI:1.00-1.26)Conclusions In spite of the influence of different regions and different type of IBD,there is an increased risk of stroke in the IBD patients comparing with the general populations, especially in the elderly patients and female patients.
