Objective : Thyroid tumor is the most common endocrine tumor, and its incidence increases year by year. Thyroid tumor has become one of the important killers affecting human health. The pathogenesis of thyroid cancer still remains unclear by far. Cytokines are small molecule proteins which play important roles in tumor immune and tumor treatment. Thyroid cancer treatment methods include chemical treatment、physical therapy and surgical treatment, and surgical treatment is the important way to thyroid cancer. Through joint detection of cytokines IL- 4, IFN-γ、IL – 10 and IL- 17 in the blood of preoperative and postoperative patients with thyroid neoplasms, we can study the cytokines expression difference before and after operation. Then analyze the relationship of cytokines expression among the different pathological types、different clinical stages、different surgical methods and different water source regions,and then study the relationship between the cytokines IL- 4、IFN-γ、IL – 10 、 IL- 17 and thyroid tumor.Methods: Selected 102 cases(34 cases with benign tumor, 68 cases with malignant tumor) of patients who underwent resection in October 2012 to November 2014 in Zhe Jiang Sian international hospital as the object, and 106 normal persons as normal controls. Record the object cases’ information. ELISA was used to test the level of four kinds of cytokines(including IL-4, IFN-γ, IL-10,IL-17) in preoperative and postoperative patients’ blood, and at the same time tested the level of IL-4、 IFN-γ、 IL-10、IL-17 in 106 cases of healthy control. Statistical analysis was performed by using the software SPSS13.0. Comprehensively analysis the relationship between the four kinds of cytokines and the thyroid neoplasm.Results: 1 The level of IFN-γ、IL-4、 IL-10、IL-17 in thyroid tumor patients and operation. 1.1 Before operation,The level of IFN-γ in benign and malignant thyroid tumor group were all obviously lower than IFN-γ in healthy controls group, and the level of IL-4、 IL-10、IL-17 were obviously higher than the level of IL-4 、 IL-10 、 IL-17 in healthy controls. P< 0.05, the difference was statistically significant. The level of IL-4、 IFN-γ 、IL-10、IL-17 in benign and malignant thyroid tumor group had no obvious difference, P>0.05.(Table1- Table 4,Fig.1- Fig.4) 1.2 After operation, in benign and malignant thyroid tumor group, the level of the cytokines IL- 4、 IL- 10、 IL-17 decreased obviously than those in preoperative patients, but the level of IFN-γ increased obviously than the IFN-γ level in preoperative patients. P<0.05, the difference was statistically significant. The level of IL-4 IFN-γ、 IL-10、IL-17 in benign and malignant thyroid tumor group all had no obvious difference with those in healthy controls. P>0.05, there was no statistically significant difference.(Table1- Table 4,Fig.1- Fig.4)1.3 The level of IL-4、 IFN-γ、 IL-10、IL-17 in thyroid tumor patients after the operation using thyroid local resection and the other two methods(thyroid full cut, thyroid gland with isthmus resection) had obvious difference, P<0.05,but between thyroid full cut method and thyroid gland with isthmus resection, there was no obvious difference, P>0.05.(Table 5- Table 8,Fig.5- Fig.8) 2 The level of IL-4, 、IFN-γ、 IL-10、IL-17 in different pathological types of thyroid carcinoma patients had no obvious difference, P>0.05. The level of IL-4, 、IFN-γ, 、IL-10、IL-17 in different TNM clinical stages had no obvious difference, P>0.05.(Table 9- Table 16,Fig.9- Fig.16) 3 The level of IL-4、 IFN-γ、 IL-10、IL-17 in thyroid tumor patients from water polluted areas and water unpolluted areas had no obvious difference, P>0.05.(Table 17- Table 20,Fig 17- Fig 20)Conclusion:1 By the result, we could see the level imbalance phenomenon of IL-4 and IFN-γ in benign and malignant thyroid tumor patients(higher level IL-4,lower level IFN-γ).The reason of this phenomenon may be related to "Th1 / Th2 drift". The level of IL-10 and IL-17 in benign and malignant thyroid tumor patients were significantly higher than those in normal control group.2 The level of IL-4, 、IFN-γ、 IL-10、IL-17 in thyroid tumor patients after the operation using thyroid local resection and the other two methods(thyroid full cut, thyroid gland with isthmus resection) had significant difference.3 The expression of IL-4、 IFN-γ、 IL-10、IL-17 in benign and malignant thyroid tumor group、different pathological types of thyroid carcinoma patients and different TNM clinical stages thyroid carcinomas all have no statistically significant difference.4 The expression of IL-4, 、IFN-γ、 IL-10、IL-17 in thyroid tumor patients had no obvious relationship with the polluted water in Jia xing region.
