Purpose:To explore the value of Hepatobiliary Seintigraphy which is used to watching the biliary tract clear or not in the assessment by radioactivity ratio between liver and heart of infants’ prolonged jaundice.Method:52 cases of infants’ prolonged jaundice underwent hepatobiliary scintigraphy and analysis the results.1 Collect the disease history of 52 cases of infants’ prolonged jaundice.2 Gather the results of function of liver and blood coagulation,witch is detected on the same day or in the recent days that doing hepatobiliary scintigraphy.3 Give a judgment about the biliary patency.And the degree of liver cell uptake was observed in children with the imaging agent.4 Analysis the date of Hepatobiliary scintigraphy results through region of interest(ROI) technology that sketching out interested in the liver, heart and kidney interested area at the 0min、5min、10min、15min、20min、30min images. Recording the count of radioactive and calculating ratio between the liver and heart, kidney. And through the ratios to assess the classification of liver damage.5 Do the assessment about classification of liver damage through Child-Pugh Evaluation System which is evaluated by clinical disease history,and Ohkuma Evaluation System that is assessed by pathology results of liver.6 Application SPSS 13.0 software for statistical analysis of research data and analysis of the data.7 Track the treatment and clinical outcomes of the patient.Results:52 patients clinical data were collected, there were 32 males, 20 females. Positive imaging in 45 cases. Seven cases were negative imaging.The negative imaging were exculded.Two positive images were also excluded because of the bilirubin levels in the blood are too high.The higher bilirubin levels would affected liver absorbing the imaging agent.Finally,29 cases who get the pathology results of liver can be analysised.Through the SPSS 13.0 software to do the analysis between the classification of Ohkuma Evaluation System and the classification measured by the hepatobiliary scintigraphy and Child-Pugh Evaluation System.The results of statistical analysis:there is a consistency relationship between the classification assessed by ratio between liver and heart and classification of Ohkuma Evaluation System(Kappa=0.549,P<0.001). The classification assessed by ratio between liver and kindey is the same(Kappa=0.438,P=0.001). The relation between classification measured by liver radioactive counts and Ohkuma Evaluation System is inconsistent(Kappa=0.420, P=0.002). The relation between Child-Pugh Evaluation System and Ohkuma Evaluation System is inconsistent(Kappa=0.387,P=0.004). The classification measured by ratio between liver and heart more accurately reflect liver damage.Conclusion:1 Hepatobiliary scintigraphy has very great significance in diagnosising children with obstructive jaundice.2 Hepatobiliary scintigraphy can quantitatively evaluate children with obstructive jaundice and thire liver function.3 The classification measured by ratio between liver and heart,kindey and liver radioactive counts are more accurately reflect liver damage and judge prognosis.
