Pneumonia in children is one of the most common infectious diseases, but also the main cause of child death. Bacteria, viruses, mycoplasma, chlamydia and other pathogens can cause pneumonia. Pneumonia mainly affects the terminals airways, alveolar and interstitial lung. If pneumonia patients can not be timely identification and treatment, likely to be severe pneumonia, cause a variety of complications or even die. Some studies indicated that inflammatory cytokines play an important role in the development of severe pneumonia. Therefore, understanding the causes of pneumonia in children and exploring its therapy have a great significance for improving the health of children. In this study, we will observe the dynamic changes of serum IL-1R1 and APC in severe pneumonia, and discuss the clinical significance in the diagnosis and treatment of disease.Objective: Serum IL-1R1 and APC level of children with severe pneumonia were detected by ELISA. Observed dynamic changes of serum IL-1R1 and APC of the patient at the first day, the third day, the seventh day, discuss the clinical significance in the diagnosis and treatment of disease.Methods:1 Experimental groups: Children with severe pneumonia who are hospitalized in Children’s Hospital of Hebei Province PICU from Jan. 2014 to Jan. 2015.According to Zhu Fu Tang "Practical Pediatrics"(seventh edition) diagnostic criteria of severe pneumonia, made the diagnosis according to medical history, clinical manifestations, X-ray, biochemical test, bacteriological test. 30 cases of severe pneumonia in children from aged 1 month to 3 years and 10 months, 19 males and 11 females,According to the time of hospitalization time divided into three groups; the control group:20 cases of healthy children, aged from 3 months to 10 years old, 9 males and 11females.2 Detecting Serum IL-1R1 and APC level by ELISA. Compared the first day, the third day, the seventh day and control group of serum IL-1R1 and APC.Results:1 The comparison of serum IL-1R1 level of the first day, third day and seventh day of hospitalized of severe pneumonia Serum levels as follows:(0.72±0.33)ng/ml,(0.40±0.19)ng/ml,(0.23±0.12)ng/ml. Compared among the three groups, the difference was statistically significant(P<0.05) after a few days treatment.2 The comparison of serum IL-1R1 levels of the first day, third day and seventh day of hospitalized and control group: levels of the first day, third day and seventh day of hospitalized and control group as follows:(0.72±0.33)ng/ml,(0.40±0.19)ng/ml,(0.23±0.12)ng/ml,(0.16±0.02)ng/ml. Compare with the control group: the first day of hospitalized serum IL-1R1 levels were significantly higher, between the two groups was statistically significant(P<0.05), the third day of hospitalized serum IL-1R1 levels were significantly higher, between the two groups was statistically significant(P<0.05), the seventh day of hospitalized serum IL-1R1 levels were not significantly higher, between the two groups was no statistically significant(P>0.05).3 The comparison of serum APC level of the first day, third day and seventh day of hospitalized: the first day, third day and seventh day of hospitalized of severe pneumonia Serum levels as follows:(167.95±70.84)pg/ml,(263.65±77.19)pg/ml,(406.48±49.59)pg/ml. Compared among the three groups, the difference was statistically significant(P<0.05) after a few days treatment.4 The comparison of serum APC level of the first day, third day and seventh day of hospitalized and control group: levels of the first day, third day and seventh day of hospitalized and control group as follows:(167.95±70.84)pg/ml,(263.65±77.19)pg/ml,(406.48±49.59)pg/ml,(405.89±101.96)pg/ml. Compare with the control group: the first day hospitalized serum APC levels were significantly lower, between the two groups was statistically significant(P<0.05), third day of hospitalized serum APC levels were significantly lower, between the two groups was statistically significant(P<0.05), the seventh day of hospitalized serum APC levels were not significantly lower, between the two groups was no statistically significant(P>0.05).Conclusion:1 Children with severe pneumonia increased serum IL-1R1 level, and with the recovery, the serum IL-1R1 level gradually returned to normal,IL-1R1 play a contrast role in the occurrence and development of pneumonia by inducing inflammation.2 Children with severe pneumonia decreased serum APC level, and with the recovery, the serum APC level gradually returned to normal,APC can reaction of inflammation of pneumonia.
