Objective:To study the relationship of oxygen utilization coefficient and prognosis after acute carbon monoxide poisoning in patients with delayed encephalopathy,provide for acute carbon monoxide intoxication delayed encephalopathy new basis for the early diagnosis and prognosis.Methods:86 patients diagnosed with acute severe carbon monoxide poisoning,Gas analysis of arterial boold and central venous blood and calcuation of O2 UC [ O2UC=( Sa O2- Sv O2) / Sa O2] were carried out on admission and 6hours,24 hours,48 hours, 72 hours after admission, And all patients on admission were collected venous blood to test of myocardial enzyme, blood sugar, carboxyhemoglobin concentration, And the pulse oxygen saturation(Sp O2), the age of patients, coma time and Glasgow coma score(GCS score) at the time of and 24 hours after admission were recorded.after two months follow-up, were divided into delayed encephalopathy group(group A) and non delayed encephalopathy group(group B).The differences of two groups about age, coma time, carboxyhemoglobin concentration, the pulse oxygen saturation, glasgow coma score, myocardial enzyme and blood sugar were compared.and the oxygen utilization coefficient in different times were analysed and statistically.Results:Delayed encephalopathy group(group A)of 21 cases and non delayed encephalopathy group(group B)of 64 cases were included.The Oxygen Utilization Coefficient of normal control group were(28.12±3.23)%.1 There were statistically difference between two groups on the age,comatime,glasgow coma score on 24 hours after admission,and there were no statistically difference between two groups on carboxyhemoglobin concentration, the pulse oxygen saturation, glasgow coma score on admission, myocardial enzyme and blood sugar.2 There are significant differences between the two groups in Oxygen Utilization Coefficient on admission(group A 54.01±10.32, group B 41.25±6.08), and apparently higher than group C( P<0.01). There are also significant differences in O2 UC at 6 hours, 24 hours, 48 hours after admission, and no significant differences in O2 UC 72 hours after admission.( P>0.05)3 The variation trend about O2 UC between two gronps in different times: O2 UC both of the two groups were decreased,in group B it returned to normal at 24 hours after admission, and then to 72 hours remained at normal levels,and it fell back slowly in group A, and the recovery time is prolonged.Conclusion:O2UC shows the anoxic state and oxygen utilization barrier of the cells. The initial level and duration of abnormal conditions of O2 UC is closely connected with Acute Carbon Monoxide Poisoning, so it is necessary to restore O2 UC as soon as possible, and improve the prognosis outcome of Acute Carbon Monoxide Poisoning.
