Objective:To explore the risk factors of pulmonary infection after gastric cancer surgery, logistic regression models to be established by the main risk factors, to provide the reference forclinical prevention and treatment of pulmonary infection after gastric cancer surgery, in order to reduce the incidence rate of postoperative pulmonary infection.Methods:This was a retrospective study, from January 1,2012 to December 31,2013 in The Affiliated Hospital of Putian College were collected the clinical data of 268 patients underwent radical operation for gastric cancer, according to the inclusion criteria and 16 were excluded, the remaining 252 patients entered the study. On the basis of postoperative pulmonary infection, divided into non complicationgroup with 214 cases, complications in 38 cases. The incidence of postoperative pulmonary infection statistics, to compare their differences in gender,age, preoperative albumin, lung function,smoking, diabetes, preoperative respiratorydisease, operation mode, operation time, blood transfusion, gastric tube indwelling time, preoperative antibiotic use time.First in 14 clinical variables was analyzed by single factor analysis, measurement data with X±S said, according with normal distribution,use t-test, if not, use the rank sum test; count data with chi-square test. Then take the variables are statistically significant with multi factors Logistic regression analysis. Statistical analysis was performed using the SPSS 18.0 software, using p=0.05 as thesignificance limit, set up Logistic regression equation.Results: In this study, there were 38 patients with postoperative pulmonary infection in 252, the infection rate was 15.08%. Through single factor analysis by the clinical indicators showed that the incidence of pulmonary infection after operation of gastric cancer concerned with sex, smoking, blood transfusion, FEV1, FEV1/FVC%, Mvv%, Albumin≤ 30g/L factors,has nothing to do with age, diabetes, preoperative respiratory disease, operation mode, operation time, indwelling gastric tube time, preoperative antibiotics use time factors. Postoperative pulmonary infection as the dependent variable Y (0=no, 1=yes), in single factor analysis, 7 factors with statistical significance: gender, smoking,blood transfusion, FEV1, FEV1/FVC%, MVV%, Albumin≤30g/L,as independent variable X,input the multivariate regression analysis method of Logistic. The results showed: there were 3 factors. into the Logistic regression equation, the intensity of the effects is as follows: the blood transfusion (OR=7.846), smoking (OR=3.390), FEV1 (OR=0.226), and obtained the Logistic regression mode:P=1/1+ ExpΣ (0.673-2.060X1-1.221X2+1.489X3).Conclusion: 1.Pulmonary infection after operation of gastric cancer is influenced by many factors, The study found that the statistics of three risk factors of pulmonary infection after gastric cancer surgery in our hospital are blood transfusion.smoking and FEV1.It has some value for future clinical and scientific research, but incomplete informations, it is necessary to design a reasonable prospective study. 2. The established Logistic regression model can be used to predict the occurrence of pulmonary infection after operation of gastric cancer, but the small sample sizeand the observation index shortage, the research has certain bias. If further research, it is hope to build a more accurate model.
