Objective:To observe the effect of dexmedetomidine combined with Pentazocine on patient-controlled intravenous analgesia after Post-caesarean.Methods: 80 patients with ASA physical I orⅡundergoing Post-caesarean were randomly divided into two groups, with 40 cases in each group. Group I Patients received 20 ml saline in 10 minutes after delivery of the baby. Their PCIA consist of180 mg Pentazocine plus 5mg Tropisetron.Group II: Patient sreceived20mldexmedetomidine(0.5ug/kg)in 10 minutes after delivery of the baby.Their PCIA consist of 180 mg Pentazocine plus Dex 300 ug and 5mg Tropisetron. The medicine was diluted in 100 ml of saline.The group I administered at a continuous dose of0.027mg/kg/ h and a bolus dose of 0.042 mg/kg.The group II administered at a continuous of Pentazocine 0.027mg/kg/h and dexmedetomidine 0.045ug/kg/h. PTh and PTTh were measured using the micro-current stimulation method before anesthesia and after dexmedetomidine administered one hours and recorded the total amount of Pentazocine consumption over the 24 h after surgery.The median time to initiation to initiation of lactation was recorded. VAS pain scores and Ramsay sedation scores at hour 4, 8, 24.and recorded the side effects and satisfaction of the patients.Results:as compared with group I, PTh and PTTh at 1 hour after bolus injection of dexmedetomidine were significantly increased in group II, The total amount of Pentazocine consumption over the 24 h after surgery in group II(83.9±6.2) was lower than in group I(112.6±9.8mg).VAS scorings was all less than 4 points. but group II lighter pain intensity than the other group(P<0.05).Ramsay scores are all 2 in each group. The side effect in group II was lower than I group. The II group patients satisfaction were higher than group I.there was no significant difference in serum cortisol concentration among the 2group.Conclusion: Patients received dexmedetomidine(0.5ug/kg) after delivery of the baby,and the combination of dexmedetomidine and Pentazocine for PCIA aftersurgery, not only reduce Pentazocine consumption,but also increase the side effects and improve patients sttisfation.
