The Effect Of Chinese Therapeutic Music-Wuxing Jiao Tune On Convalescent Stroke Patients With Depression

Objective To probe the rehabilitation effect of improving depression through the implemen-tation of Chinese Therapeutic Music-Wuxing Jiao tune on convalescent stroke patients with depression,observing the changes of the Hamilton Depression Scale scores(HAMD),serum 5-hydroxytryptamine(5-HT).Providing a convenient,effective and affordable rehabilitation nurs-ing programme for convalescent stroke patients with depression.Methods 74 hospitalized convalescent stroke patients with depression of rehabilitation dep-artment were randomly divided into the tune of Jiao and the Western music group of 37 cases each.Both groups accepted the conventional treatment.Furthermore,the tune of Jiao group list-ened Chinese Therapeutic Music-Wuxing Jiao tune,the Western music group listening Weste-rn music.Listening time lasted from 90 to 100(minutes,min) every time,seven times a week f-ollowing 4 weeks.Before and after intervention,We observed the scores of 17 version of Ham-ilton Depression Scale(HAMD-17),detecting serum 5-hydroxytryptamine(5-HT) changes.Results1. Baseline data comparative analysis of two groups before the testBefore the test,there was no statistical difference in the general information[family origin ,age,course of disease,sex,marital status,type of stroke,lesion site,the number of lesions,nearly a vear whether the stroke surgery,the freauencv of stroke,paralvsis side,level of education, wh-ether accompanying relatives,motor function(Fugl-meyer upper and lower limb motor functi-on scores),cognitive function(MOCA scores),language function(China rehabilitation research center aphasia examination,CRRCAE),dysphagia function(Swallowing disorder clinical insp-ection and record),whether associated with brain atrophy] and baseline evaluation index(HA-MD total scores and sub-scores,serum 5-HT) between the two groups of patients(P>0.05).2. Compliant analysis of the intervention on PSD by MusicThe test completion status analysis of of two groups showed that there was 1 case of the tune of Jiao group and 2 in the western music group failed to complete the test scheme,who were eliminated during the test.The average time length of listening prescribed music in the t-wo groups was not different(P>0.05) with totally 93.99±4.20 min in the test plan 90～100 min every day.It showed the overall good compliance.It displayed no difference in time length of additional viewing/listening to music programs/songs between the two groups(P>0.05).3. HAMD scores analysisChinese Therapeutic Music-Wuxing Jiao tune was more effective than Western music in HAMD efficacyafter 4 weeks’ intervention(P<0.01).In the fourth week after the test,the HA-MD scores of the tune of Jiao group were less than those of the Western music group(P<0.05) We adopted repeated measures analysis of variance to analyze the difference of HAMD score-s in the two groups before the test and the first,second,third,fourth week after the test,Which showed that there was no difference between the two groupsin HAMD total scores consideri-ng the main effect of grouping(P>0.05).In the main effect of time factor,the two groups of H-AMD total scores changed as time went by(P<0.05).Grouping and time effect interacted(P<0. 05).Grouping and time factors interaction trend chart showed that compared to the Western music group,the tune of Jiao group had better efficacy in reducing HAMD total scores under the influence of time factor.Further comparison showed that the scores of systemic and genital symptom were lower in the tune of Jiao group in the second,third,fourth week.4. Serum 5-HT content analysisThere was higher serum 5-HT content after the test than before in the tune of Jiao group( P<0.01),while without statistical difference between before and after the test in the western music group(P>0.05).After the test,serum 5-HT of the patients in the tune of Jiao group was higher than that of the Western music group(P<0.05).The tune of Jiao group’ 5-HT content in-creasing,while systemic symptom and genital symptom scores of HAMD lower than the Wes-tern music group after the test suggested that one of the possible mechanisms to improve portion of depressive symptoms better than Western music by Chinese Therapeutic Chinese Therapeutic Music-Wuxing Jiao tune was to increase 5-HT content.5. Safety ObservationSafety observation showed that the two groups’ patients didn’t have severe adverse react-ions and side effects,which prompted listening music for PSD patients was safe and feasible.Conclusion1. The overall improving depression effect of the tune of Jiao group is superior than the Western music group as time goes by with better efficacy in reducing the scores of the syste-mic symptom and genital symptom of HAMD.2. Chinese Therapeutic Music-Wuxing Jiao tune can increase serotonin(5-HT) content of convalescent stroke patients with depression,which may be one of the possible mechanisms to produce better improvement in portion of depressive symptoms than Western music.3. Chinese Therapeutic Music-Wuxing Jiao tune has high safety and good compliance in intervention of convalescent stroke patients with depression.
