Objective The study was designed to use the questionnaire of bNRI and audiovisual facilitated tbNRI to examine the close relationship interactive qualities of the adolescent in hospital, and to further explore whether there are hidden phenomenon and gender difference in self-report. It was used to provide the support for the development of more effective assessment tool of close relationship, as well as to lay the foundation of providing peer emotional support and related intervention to adolescent in the future.Methods A cluster sampling method was used, from July 1st 2012 to March 31st 2013,89 inpatients in ENT wards in a provincial general A level hospital who met the inclusion criteria were included. Using a coin toss randomly divided the participants in to experimental group and control group,44 cases and 45 cases respectively. On the day of admission, all of the participants need to answer the self-reported questionnaire of adolescent health and development, including the pubertal development, the knowledge, attitude and behavior of puberty, family function, physical dysfunction, close relationship and cognitive development. At 14:00-17:00 on the second admission day, the participants in experimental group need to finish the audiovisual facilitated tbNRI test. IBM SPSS Statistics 19 for Windows was used to perform descriptive, chi-square (x2) tests, independent sample t-tests, correlational and reliability analyses for demographic information, distribution of disease and subjective measurement results.Results There were 7 cases who have studied in college or worked been excluded, the final analysis included 82 cases with experimental group of 40 and control group of 42. Among them there are 53 boys(64.6%) and 29 girls(35.4%), the average age is 15.3±2.99, and the average BMI is 20.5±2.65 kg/m2. Upon examination, there was no significant difference between experiment and control group in various indicators of social demographic, health and development assessment. The results were as follows.1. The score of puberty-KAB on average was 3.6 ±0.50, which was between "3" and "4", indicating that participants have certain puberty related knowledge, attitude and behavior. The average score of ZCDI was 3.8±0.62, the score of Education Relative and Interpersonal Relative were higher than Education Bipolar and Interpersonal Bipolar, indicating that the participants can think dialectically. The average score of WHODAS 2.0 was 1.8±0.54, between "1" and "2", which means the health dysfunction degree was low.2. In the bNRI direct test, there were 19.5% (16/82) participants showed hidden phenomenon in answering the "boyfriend/girlfriend" related questions, indicating that they may hold avoiding or concealing attitude when answer the questions. Further analysis found that there was no gender difference, yet age difference, and the phenomenon was more significant in primary school students. The result of tbNRI indirect test, only 1 case showed the hidden phenomenon, compared with the 8 cases in direct test, has been greatly improved.3.81.7% participants thought that the same-sex friend was their best friend, and only 22% participants took opposite-sex friend as their best friend in the question of the closeness degree of opposite-sex friend, and the gender difference was not significant. Further analysis found that, there were 18 cases reporting that they didn’t have opposite-sex friend, among them 9 were primary school students and average age was 13.3±2.79.4. The results showed that in the hospitalized adolescents’ relationship with same-sex friend, opposite-sex friend and boyfriend/girlfriend, positive interaction scores were higher than negative interaction, while in the comparison of three relationships, same-sex friend showed most positive and negative interactions, while the positive tendency is larger than negative tendency. In the test of tbNRI, "Carl" relationship’s positive and negative interaction was the most similar with the "same-sex friend" in the direct test.5. The Pearson correlation results showed that, in positive interaction factor:there were correlations between same-sex friend and opposite-sex friend, Carl, Mike and Lina, and the correlation degree was higher between same-sex friend and opposite-sex friend (a=0.711). There were correlations between opposite-sex friend and Mike, Lina. Among Carl, Mike and Lina there existed correlations. In negative interaction factor:there were correlations between same-sex friend and opposite-sex friend, Carl, Mike, and the correlation degree between same-sex friend and Carl (a=0.756), Mike(a=0.789) were high. The correlations between opposite-sex friend and Carl, Mike were low. The correlation between Carl and Mike was strong (a=0.786).Conclusion The positive interactions are more than the negative interactions in the hospitalized adolescents’ interpersonal communication. The hidden phenomenon existed in the self-report, yet gender difference was not significant. Most adolescents took same-sex-friend as the best emotional support partner, indicating that nurses should get the knowledge of how to assess the close relationship network, and set up peer interaction condition by taking advantage of related tools to give effective emotional support to adolescents.
