Objective:To study the clinical characteristics, Diagnosis, treatment and prognosis of spermatic cord leimyosarcoma.Methods:A retrospective analysis was performed on one case of spermatic cord leiomyosarcoma treated in the urology department of Shandong provincial hospital. The patient, male,68, was diagnosed with right spermatic cord tumor by ultrasonography; No lymphatic metastasis was found by PET-CT; AFP:4.71ng/ml, CEA:2.37ng/ml, PSA:1.570ng/ml. The patient finally underwent complete surgical resection of the tumor in right scrotal and radical unilateral orchiectomy. With this clinical data by literature review we study the clinical characteristics, Diagnosis, treatment and prognosis of spermatic cord leimyosarcoma.Results:The patient underwent complete surgical resection of the tumor in right scrotal. During the operation, the complete surgical resection was performed and the biopsy has been made in the operations, which showed spindle cell tumor, considering leiomyoma. While the postoperative pathological diagnosis showed spermatic cord leiomyosarcoma(Grade I), so we then performed radical unilateral orchiectomy for the patient two weeks later. As the PET-CT found no obviously positive lymph node, we didn’t perform RPLND for this patient. The patient had an uneventful recovery without chemotherapy or radiotherapy. No recurrence and metastasis have been found after 3 months followup.Conclusions:Spermatic cord leiomyosarcoma is rare, the main clinical manifestation is painless mass. Frozen section analysis may be misdiagnosed, the final diagnosis is based on postoperative pathologic results. Radical resection is the first choice of treatment. Postoperative radiotherapy can improve the prognosis and reduce the recurrence.
