| ObjectiveThe incidence of hospital infection in this hospital was investigated from 2010 to 2012, to explore the epidemiology characteristics and infection,hospital infection factors,for the prevention and treatment recommendations.Method1.The selection patients of 2010 January to 2013 December were analyzed, the present situation of hospital infection. were investigated in the last four years.2. Exclusion patients:whom existed pre hospital infection; inflammation due to trauma; neonatal placenta obtained such as chicken pox,herps disease and Toxoplasmosis,infection inpatients with preexisting chronic infection acute attack.3.Cross-sectional study to investigate the unified national hospital infection management training monitoring A1 Qaeda infection in the Ministry of health hospital. According to sections each of 50 beds with 1 medical personnel to carry out the investigation,these medical personnel required for infection contrl practitioner with the attending physician title.The medical personnel should be trained to unserstand:diagnostic criteria,surver method,questionnaire items. Medical personnel in the survey should pay attention to reord the temperature record,antimicrobial use reason, admission diagnosis, pathological examination result.4. The software SPSS13.0 was used for data analysis, X2 test was used for qualitative data analysis, when P<0.05, showed that the difference has statistical significance.Result1.From 2010 January to 2013 December,the investigation on 20138 hospitalized patients, the hospital infection pccurred in 5689 patients, the average infection rate was 2.82%, the annual incidence of NI were 3.21%,3.11%,2.82%,2.38%. Four years of infection rate were compared,2010 and 2011 P value>0.05, showed that was no difference in infection rate of these two years.The other years compared to P<0.05, showed that the infection rate was different. The incidence of hospital infection did not increase with the number of cases, there is a downward trend, the well control the infection in the hospital.2. The main part of comprehensive hospital infection found the hospital infection site had 1320 cases, accounted for 23.21%, lower respiratory tract infection; followed by 746 cases of urinary system infection accounted for 13.11%,729 cases of upper respiratory tract infection accounted for 12.82%, gastrointestinal infection accounted for 9.38%,534 cases of skin and soft tissue infection had 468 cases, accounted for 8.23%, operation site infection accounted for 6.43%,366 cases of blood infection accounted for 5.45%,310 cases of oral infection in 248 cases, accounted for 4.36% of the basin (ventral) 215cases accounted for 3.78%, other 753 cases of infection accounted for 13.23% cavity infection.3.The every department of NI distribution found that the incidence of hospital infection among different departments have differences,the top three departments were central ICU, department of hematology, internal medicine-neurology, followed by the deparyment of burns deparyment(6.13%),deparyment of general surgery (5.25%),department of internal medicine (5.12%), department of cerebral surgery (5.01%),nephrdogy department (4.43%), department of chest surgery (4.42%), department of gynaecology and obstetrics (4.41%), department of urology (4.32%) department of endocrine (3.21%),department of respiration (3.12%), department of oncology (2.09%), department of orthopedics (2.08%) department of dermatological (1.68%), department of paediatrics (1.67%), department of stomatology (1.09%), department of cardiology (1.08%), ENT (0.87%), department of ophthalmology (0.65%)4.5869 patients submittde secretion samples of hospital infection, a total of 17396 strains of bacteria were isolated, including 9524 strains gram negative bacilli accouned for 54.75%, top three are Bauman Acinetobacter, Escherichia coli, Klebsiella pneumoniae accouned for 18.73%,16.99%,8.17%; 6967 strains of gram positive cocci accouned for 40.05%,the top three were Enterococcus faecalis, Staphylococcus aureus, Staphylococcus haemolyticus, accouned for 9.65%,8.88%,6.82%; 873 strains of fungi accounted for 5.02%,the top three were Candida albicans,Candida tropicalis,Candida glabrata strains. Accounted for 2.22%,1.21%,0.90%.Among them the pathogenic bacteria isolated from the top 10 were Bauman Acinetobacter spp, Escherichia coli, Enterococcus faecalis, Staphrlococcus aureus, Klebsiella pneumoniae, Staphrlococcus haemolyticas, Pseudomonas aeruginosa, Slaphylococcus epidermidis, Streptococcus pneumoniae, Candida albians.5. The risk factors of hospital infection and the age, underlying diseases, there is a disturbance of consciousness, invasive operation, antibiotic prophylaxis, Department, operation, duration of hospitalization, age of patients with more than 60 years, there were 3185 cases of infection in the investigation of the 56698 patients, the infection rate was 5.62%, less than 144687 at the age of 60there were 2504 cases of infection, the infection rate was 1.73%. On whether patients with underlying disease, infection occurred in 1598 cases among the 37492 patients with underlying diseases, infection rate was 4.26%.163893patients had no underlying diseases in 4091 cases of infection, the infection rate was 2.50%. In 27591 cases, there is a disturbance of consciousness of patients,2855 cases of infection, the infection rate was 10.35%,2834cases of patients with hospital infection in 173794 cases and no conscious disturbance patients, the infection rate was 1.63%; 72289 cases were invasive operation in patients with 3757 cases of infection, the infection rate was 5.20%, no 129096 invasive operation patients, infection in 1932 cases, the infection rate was 1.50%. Prevention of infection occurred in 87996 cases of drug use of antibacterial in patients with 3793 cases, the infection rate was 4.31%,113389 cases occurred without the use of antibiotics in patients infected with 1896 cases, the infection rate was 1.67%. For the treatment of 28656 patients with no socomial infection in 2379 cases of ICU and Department of Hematology, the infection rate was 8.30%,and 178729 cases of otner department of nosocomiai infection in patients with 3310 cases, the infection rate was 1.92%.75679 cases of operation patients infect with 3359 cases, the infection rate was 4.44%,125706 cases without operation in patients with 1330 cases of infection, the infection rate was 1.06%,95980hospitalization over 30days in patients with 3689 cases of infection, the infection rate was 3.84%, the hospitalization timeless than 30 of the 105405 patients the incidence of infection,2000 cases of patients, the infection rate was 1.90%. The above statistical by the chi square test to obtain the P value <0.001, showed that the difference has statistical significance. The full proof of age over 60 years old, with basic diseases, there is a disturbance of consciousness, invasive operation, prophylactic use of antimicrobial drugs, Central Department of Hematology operation, hospitalization time, ICU>30 days are the risk factors of nosocomial infection. ConclusionHospitalized patients with nosocomial infection in hospital is inevitable, the establishmentof an effective control system is effective in preventing and reducing the occurrence of hospital infection important guarantee rate. |
