BackgroundWith the transformation of medical model, great changes have taken place in people’s health demand, no matter in content or in the nature and scope, higher request to the physician’s practice ability is put forward. Physicians are the core of hospital’s competitive resources, How to evaluate and improve medical clinical practice ability? And how to revitalize the existing human resource, arouse the enthusiasm of physicians? Professional design for physicians, completing the performance distribution for the managers and going out of the title assess error, providing excellent, efficient and safe medical services for patients, quantitative indicators for evaluation is needed, which can guide the continuous improvement for physicians along the correct route of the hospital’s reform and development.In our country, quantitative evaluation index system of physician practice ability has not yet been formulated, the assessment content on physicians regular examination is general, to some degree, it is a mere formality. To promote the tertiary hospital doctors guided by the character, ability, and the performance, strengthen the self management, continuously improve service, improve work enthusiasm, we should develop a scientific and reasonable quantitative evaluation index system of physician practice ability as soon as possible. It is of practical significance to guide physicians pay attention to the connotation of the service and quality, promote actively adapt to the reform of public hospital, improve the core competitiveness, and improve hospital management level. Based on these, we investigate on the building of quantitative evaluation model of ability to physicians in clinical practice.ObjectivesThe main purpose in this study is to establish a scientific, practical, and easy to be operationed quantitative evaluation index system of the physician’s practice ability, build the quantitative evaluation model, from perspective of the physicians individual practicing ability management, accurately and objectively evaluate the ability of clinical practice, investigate the establish of assessment tools and mechanisms matching the clinician practice post management, to make " Outstanding physicians " stand out, and find the weak in the process of practice, find out gap and take the initiative to adjust to improve, stimulate work enthusiasm of physicians, to provide guidance for continued ascension physicians practicing ability, and do a useful exploration to establish a more scientific and reasonable evaluation index system of physician practice ability.MethodsLiterature study, expert interview method, Delphi method and analytic hierarchy process (AHP), and other statistical methods were used in this study.Based on the literature, the physician’s practice ability quantitative evaluation index system framework model, as well as various indicators of dimensions and items, were constructed, the questionnaire to the established index system framework was designed.Using the Delphi method,30 experts were counseled with face-to-face or by E-mail questionnaires, after two rounds,4 second grade indexes and 32 third grade index of quantitative evaluation index system of physician practice ability were identified. All levels of indexes’scientific empowerment were determined by analytic hierarchy process (ahp), and the weights and the combination weights of second grade and third grade index were established.Based on the statistical analysis of the active coefficient of experts, authority coefficient and coefficient of expert opinion concentration, coordination, the Delphi method’s representative and credibility were checked, the weight was set by analytic hierarchy process (AHP), consistency was checked, evaluation model was set up, all the data were input and calculated with Excel, the statistical data were analyzed with SPSS statistical software..Main ResultsFour dimensions of evaluation index system of the work performance, business ability, learning and professional ethics were built, the physician’s practice ability evaluation system that including 4 second grade indicators and 32 third grade indicators was determined ultimately.Expert consultation results:(1) active degree:two rounds of questionnaire recovery rate were 96.7%,100% respectively. Efficient was 100%, active degree of experts was high; (2) the degree of authority:degree of experts’familiarity and degree of judgment were 0.91096,0.9077, the degree of authority was 0.9093;degree of authority was high; (3) coordination degree of expert opinion:expert opinion coordination coefficient of the second grade and third grade indicators were 0.144 and 0.256, with statistically significant (P< 0.05) by statistical tests, suggesting that the expert opinion coordination was focused, the results were available; Cronbach’s Alpha coefficient of two rounds of expert consultation scale were 0.8437,0.8522, which were both higher than 0.8, which had high internal consistency, high reliability, the results of evaluation with the index system matched physician’s practice, the result was reliable.The weight coefficient of the second grade and third grade indicators was determined.4 second grade indicators were work performance, business level, learning and professional ethics, the weight coefficient of them were 0.2783,0.4681, 0.1535,0.1001; Using analytic hierarchy process (AHP), the weight and combination weight of third grade index were established. With consistency inspection of each layer of CR< 0.1, it indicated that the weight coefficient can reflect the importance of each index.Conclusions and suggestionsThe index system of physician practice ability evaluation of this model was set up, which included performance, business level, learning and professional ethics,4 second grade indexes and 32 third grade indicators. The designing of indexes was scientific and reasonable, the maneuverability was strong, the weight coefficient was reasonable, it can be used in physician practice ability evaluation in the tertiary hospital.Due to research of evaluation model the physician’s practice ability examination is still at its early stage, the determination of its index system and weight coefficient remains to be in the future empirical research, it is suggested that thorough research and gradual improvement is needed in the future work.
