Objective:To implore the clinical efficacy of Shipixiaozhi decoction and observed the mechanism and effection of shipizhig an decoction of male mice NASH induced by high fat diet and tetracycline intrape ritoneal injection to provide theoretical and experimental basis for the clinical appli cation.Method:1.Clinical research: 56 patients with NASH were randomly divided into 2 groups,Spleen Cellulite treatment group taking a daily soup,The control group polyene phosphatidylcholine capsules, 456mg/time,3 times/day,Treatment for 3months.Changes observed in patients with serum ALT, AST, TG, liver/spleen CT value, and other signs and symptoms.2.Experimental study:male kunming mice were random Ly divided into 6group.Modeling Method:The normal group were fed with common feed,while the other were used high-fat diet and tetracycline intraperitoneal injection to duplicate NASH male mice model for 7 weeks.Administration:From the beginning of the modeling,Each mice was gavage once a day for 6 weeks.Target examination:serum ALT,AST and the TC,TG Content.Weighing each liver and staining the wax sections of liver tissues by HE.The classifications of hepatic fatty degenerations and inflammations using immunohistochemical staining method detecting hepatic heme oxygenas-1(HO-1)expression.Result:The levels of serum ALT and TG in treatment and control group,declined significantly after therapy(P<0.05), while the former groups were superior to the latter group(P < 0.05) such as heavy sensation in the limbs and body,hypochondriac distention and pain, abdominal fullness and distention,bad gas color(P<0.05).Compared with the model group, the body weight of the high Shipixiaozhi decoction group was significantly reduced.The weight and index of liver: the liver weight and index model group have significantly increased(P < 0.01) compared with the normal group, but the liver weight and index of the Shipixiaozhi decoction high dose group was significantly lower than the model group(P<0.01); and polyene phosphatidylcholine group onlyhigh-dose group was significantly different from liver index(P<0.05).The serum of ALT and AST: Compared with the normal group, model group ALT,AST concentrations are significantly increased, there is a significant difference(P<0.01); but the serum of ALT and AST of the Shipixiaozhi decoction high dose group was statistically significant lower than the model group(P<0.01); the serum of ALT and AST of Shipixiaozhi decoction high dose group was significantly lower than the polyene phosphatidylcholine group(P<0.05), the Shipixiaozhi decoction dose group and more ene phosphatidylcholine group only significantly lower AST than the polyene phosphatidylcholine group(P<0.05).The serum of TG and TC: Compared with normal group, a significant increase in serum TG mice and water model of the average TC(P<0.01). Compared with the model group, the serum TG levels of high group were significantly decreased(P<0.01); TC content also decreased significantly(P<0.01).The extent of liver steatosis,lobular inflammation, hepatocyte ballooning,changes in liver fibrosis: small vacuoles in liver cells appear steatosis in the low dose Shipixiaozhi decoction group and the polyene phosphatidylcholine group; the mouse liver of model group appear diffuse steatosis 、 inflammatory cell infiltration and slight fibrosis. Soup the Shipixiaozhi decoction of the high dose group and middle dose group, then the steatosis、inflammatory cell infiltration and necrosis have been reduced significantly. Compared with the normal group, model group was statistically significant(P < 0.01); while compared with the model group, the high dose Shipixiaozhi decoction group was statistically significant(P<0.01).HO-1 immunohistochemical staining results: and the model group relatively high medicine dose group was significantly(P<0.01), traditional Chinese medicine and polyene phosphatidylcholine medicine group dose group differences was statistically significant(P<0.05); high dose group of the Polyene phosphatidylcholine group difference was statistically significant(P<0.05).Conclusion:1.The clinical research find that the level of ALT and TG in serum reduced but the liver/spleen CT values incresed for the Shipixiaozhi decoction group and the clinical symptoms have obviously improved This show that it is superior to the western medicine control group.Shipixiaozhi decoction has the role ofprotecting liver, reducing fat and fatty liver and have not found any adverse reactions.So it is a safe and effective recipe to cure NASH in the clinic.2.Shipixiaozhi decoction can effectively reduce the weights of liver, liver index,the level of ALT,AST,TG,TC in serum and improve the pathologic histology changes,it has a significant effection in resisting fatty live.3.Shipixiaozhi decoction can effectively induce the expression of HO-1 protein in mice.We also find that it has a highest expression in the high dose group and their mechanism of action in the treatment of NASH may be related to the HO-1 anti-stress and antioxidant effects.
