Background: Primary nephrotic syndrome is a common immune inflammatory disease, However, the development and severity of PNS is Closely related to renal tubular interstitial injury. Tumor necrosis factor-like weak inducer of apoptosis(TWEAK) is a cytokine of the TNF superfamily that activates the Fn14 receptor. TWEAK and Fn14 are greatly upregulated during kidney injury, promote tubular cell proliferation,induce tubular cell apoptosis under proinflammatory conditions through NF-κB activation. TWEAK plays an important role in the incidence and development of interstitial inflammation in renal.The available evidence suggests that TWEAK may provide diagnostic information and be a therapeutic target in renal injury. Its role in human kidney disease should be further explored.At present, there is no study about TWEAK relationships with primary nephrotic syndrome At home and abroad, we will detect the expression of primary nephrotic serum and kidney tissue of patients with TWEAK, first reported TWEAK and relevance of primary nephrotic syndrome the study.Objective: Determined the concentration of TWEAK expressions of serum and renal tissue in primary nephrotic syndrome.Explore the role of TWEAK in primary nephrotic syndrome pathogenesis.Method: From January 2013 to January 2014, 44patients( PNS group) who were initial diagnosed with primary nephrotic syndrome were randomly selected, coupled with 20 cases of healthy volunteers as normal control group(NC group).Collect the two groups of patients with gender, age, serum albumin(AIB), cholesterol(Chol), triglyceride(TG), low density lipoprotein(LDL), high density lipoprotein(HDL), urinary protein, C-reactive protein and other relevant clinical data, Serum TWEAK levels were evaluated by enzyme-linked immunosorbent assay(ELISA). TWEAK expression in renal tissue of PNS patients observed by immunohistochemical staining, while tested semi-quantiative score of PNS nephropathy tubulointerstitial pathological damage. Collect relevant clinical data of the two groups for statistical analysis.Result:1. The concentration of TWEAK was significantly higher in PNS group than in NC group. The difference between the two groups were statistically significant(P<0.01).2. Pearson correlation analysis showed that TWEAK were significantly correlated positively with urinary protein(P<0.05), low-density lipoprotein, negatively with serum albumin(P<0.05).3. Semi-quantitative results showed that: the overall integrated absorbance(A) values of PNS group is 25769.96±22961.01, the average A values of 0 point group is 5331, the average A values of 1 point group is 15766.44, the average A values of 2 point group is 15956.33,the average A values of 3 point group is 57401.83; The more heavy renal tubular damage, the greater the A value of TWEAK in kidney tissue.4. Spearman correlation analysis showed: The integral absorbance value of TWEAK in PNS renal tissue were positively correlated with tubulointerstitial semi-quantiative score(r=0.773,P=0.000),were negatively correlated with Serum TWEAK(r=-0.500,P=0.015),the tubulointerstitial semi-quantiative score were positively correlated with urine protein(r =-0.489, P = 0.018).5. Multiple linear regression analysis showed: there is linear regression relationship between TWEAK A value of the kidney tissue and tubulointerstitial semiquantitative score.Conclusion:1. Serum TWEAK upregulated expression in primary nephrotic syndrome.2. In primary nephrotic syndrome renal tissue, TWEAK were positively correlated with tubulointerstitial semi-quantiative score.3.
