Objective To detect the resistant situation of extended spectrum beta-lactamases(ESBL) positive strains Klebsiella pneumoniae and Escherichia coli to commonly used antibiotics in zichuan area, study of commonly used antibiotics on the prospect of these two kinds of bacteria in the area.Methods 511 K. pneumoniae strains and 403 E. coli strains that were isolated from August 2013 to October 2014 in zichuan area hospital were collected. The commonly used antibiotics MIC value of those collected strains was measured using the broth micro-dilution test, which is the clinical and Laboratory Standards Institute (CLSI) recommended ESBL phenotypic confirmatory experimental method. The judgment medicine sensitive result uses CLSI (Clinical and Laboratory Standards Institute) in 2012 the M100-S22 standard. Carries on the statistics using the Whonet software to the examination results, The x2 test was applied to statistically analyze resistance rate of ESBL-positive K. pneumoniae and E. coli as well as ESBL-negtive K. pneumoniae and E. coli to commonly used antibiotics.Results There are 511 strains of K. pneumoniae,151 of the 511 K. pneumoniae strains were ESBL positive,the positive rate was 29.55%, the resistance rate of commonly used antibiotics were in the order of ceftriaxone 96.69%, amoxicillin/clavulanic acid 32.45%, cefoxitin 17.88%, ciprofloxacin 31.79%, cefepime 17.22%, cefazolin10.00%,gentamicin64.24%, imipenem 0%,360 of the 511 K. pneumoniae strains were ESBL negtive, the resistance rate of commonly used antibiotics were in the order of ceftriaxone 7.78%, amoxicillin/clavulanic acid 9.44%, cefoxitin 10.00%, ciprofloxacin 8.61%, cefepime3.33%, cefazolin 13.61%, gentamicin 6.39%, imipenem 0%. There are 403 strains of E. coli,262 of the 403 E. coli strains were ESBL positive, the positive rate was 65.01%, the resistance rate of commonly used antibiotics were in the order of ceftriaxone 99.24%, ciprofloxacin 87.79%, cefepime 50.76%, cefoxitin 38.55%, amoxicillin/clavulanic acid 27.48%, cefazolin 100.00%, gentamicin 56.49%, imipenem 0%,141 of the 403 E. coli strains were ESBL negtive, the resistance rate of commonly used antibiotics were in the order of ceftriaxone 12.06%, ciprofloxacin 45.39%, cefepime 2.13%, cefoxitin 15.60%, amoxicillin/clavulanic acid 18.44%, cefazolin21.99%, gentamicin 39.01%, imipenem 0%.In addition to imipenem,the other antibiotics were analyzed by x2 test indicated that the resistance rate of ESBL positive K. pneumoniae to commonly used antibiotics was higher than the resistance rate of ESBL nigtive K. pneumoniae to commonly used antibiotics, and the resistance rate of ESBL positive E. coli to commonly used antibiotics was higher than the ESBL nigtive E. coli strains. All of the P value are less than 0.05 that suggested the statistically significant.Conclusion The resistance rate of ESBL positive K. pneumoniae and ESBL positive E. coli to commonly used antibiotics were higher than the resistance rate of ESBL nigtive K. pneumoniae and ESBL nigtive E. coli to commonly used antibiotics,and their drug resistance rate were higher,so the clinical should according to the drug sensitive test of rational use of antibiotics against the ESBL positive strains.
