The Clinical Study Of Jiannaoanshen Granules Treatment Of Post-stroke Depression(Heart-kidney Imbalance)

Objective:Through the comprehensive analysis and in-depth study of the literature on post-stroke depression (PSD), a clinical observation was made to evaluate the clinical efficacy and safety of Jiannaoanshen Granules treating post-stroke depression (heart-kidney imbalance) with a randomized controlled trial, and to lay a solid foundation for the further development of clinical research.Methods:This study selected 60 post-stroke depression patients which met the inclusion criteria. According to stochastic tables, the 64 cases were randomly divided into two groups:treatment group(32 cases)and control group (32 cases). Based on conventional therapy in Western medicine, the treatment group was treat with Jiannaoanshen Granules, and the control group was treat with Fluoxetine Capsules. Before treatment and after 4 and 8 weeks of treatment, the doctors attended relevant training tested for the Hamilton Depression Rating Scale (HAMD), TCM symptoms rating scale, neurological deficits Rating Scale (SSS) and daily living scale (ADL) Barthel index assess, and then calculated the HAMD reduction rate of the two groups of patients before and after the treatment. A database was established to make statistical analysis for those scale scores above by the SPSS17.0 statistical software, and then made an objective evaluation for the the ultimate effect of two groups. In addition, before treatment, after 4 and 8 weeks of treatment, the two groups of patient’s blood, urine, stool, liver and kidney function, function of blood coagulation and ECG were monitored, and the adverse reactions were recorded through the whole research process, and then the safety of two drugs were evaluated.Result:The study found that the HAMD scores and TCM symptoms scores of two groups began to decline after 4 weeks of the treatment, and after 8 weeks of the treatment it decreased significantly, which indicating that two drugs have significant therapeutic effect. Assessing clinical efficacy by HAMD reduction rate, the total effective rate was 84.38% in the treatment group, while it was 75.00% in the control group, indicating that the Jiannaoanshen Granules was significantly better than the fluoxetine. Terms of the efficacy of TCM symptoms, the total efficiency was 87.50% in the treatment group, while it was 71.88% in the control group, indicating that Jiannaoanshen Granules was better than the fluoxetine capsules to improve TCM symptoms. Comparing the above efficacy of the two groups, the difference was statistically significant (P<0.05). Moreover, Jiannaoanshen Granules has obvious advantages to promote recovery of neurological function and improve activities of daily living compared with Fluoxetine Capsules. In all the study, the urine, stool, liver and kidney function and ECG were normal in the treatment group, and nor any adverse reactions or adverse events occurred in this group, while 5 patients had nausea, vomiting, loss of appetite, constipation and other side effects in the control group, which indicating that Jiannaoanshen Granules have good security.Conelusion:Jiannaoanshen Granules is better than Fluoxetine Capsules to improve depressive symptoms and promote the recovery of neurological function and improve activities of daily living. Moreover Jiannaoanshen Granules is safer and has fewer side effects. Under the guidance of the idea of traditional Chinese medicine, Jiannaoanshen Granules is worth treating depression after stroke (heart-kidney imbalance)in clinical widely.
