Background Coronary heart disease(CHD) has been a hot research of cardiovascular disease. In recent years, there is a increasing trend in the number of patients. Therefore early detection and diagnosis has important implications for the treatment and prognosis of the disease. Echocardiography for its convenient, economical, non-invasive, etc, has become an important means of checking for heart disease. It can not only analyze changes in cardiac structure in patients with CHD and can also analyze systolic and diastolic function. This is very important to the treatment and prognosis of CHD patients. Two-dimensional speckle tracking imaging(2DSTI) technology can analyze and measure myocardial strain and strain rate. Overall wall motion parameters can reflect the overall movement of the myocardium, 2DSTI technology provides a new quantitative tool to the evaluation of myocardial function. As a noninvasive to the screening for coronary heart disease, it has become a hot research. The study used 2DSTI technology to detect CHD patients’ left ventricular overall longitudinal and circumferential strain parameters. To evaluate left ventricular systolic function and its relationship with the extent of coronary artery disease.Objective To evaluate left global ventricular systolic function in patients with CHD by using two-dimensional speckle tracking imaging(2DSTI), and to explore therelationship between left global ventricular systolic function and the degree of coronary artery disease.Methods Collected 51 patients with coronary heart disease living in cardiovascular department in our hospital was confirmed by coronary angiography from September 2013 to May 2014 as a CHD group(n=51), 33 males and 18 females, aged 38 ~ 78(61.86 ± 9.27) years old. Negative angiographic results as a control group(n=30). According to the Gensini score, the CHD group was divided into mild cases of group( <25 points), moderate cases of group(25~49 points) and severe cases of group( ≥50 points). Left ventricular global longitudinal strain(GLS) and left ventricular global circumferential strain(GCS) were measured respectively by 2DSTI. Compared the differences between groups and analyzed the diagnostic value. The parameters and Gensini score were used to carry on correlation analysisResults 1.Each group in CHD group the left ventricular ejection fraction(LVEF) less than in the control group(P <0.01), and decreased progressively with the severity of coronary artery disease.2.Compared with control group, GLS,GCS decreased in CHD group(P﹤0.05). With the severity of coronary artery disease, GLS and GCS decreased progressively(P﹤0.05).3. The receiver operating characteristic(ROC) curve showed the sensitivity of GLS,GCS in the diagnosis of CHD was 94.1%,66.7%,while the specificity of them was 66.7%,96.7%.4.Correlation analysis showed that GLS,GCS negatively correlated with Gensini score(r1﹦-0.70,P﹤0.01;r2﹦-0.67,P﹤0.01).Conclusion 1.Myocardial contractile function in patients with coronary artery disease were damaged to varying degrees, 2DSTI can evaluate left global ventricular systolic function quantitatively,and has a very important clinical significance to the patient’s diagnosis and treatment.2.Left ventricular systolic function in patients with CHD associated with coronary artery disease, and aggravated with the severity of coronary artery disease...
