Objective: To analyze the differences of spinopelvic sagittal morphology between isthmic and degenerative lumbar spondylolisthesis patients. Methods: 42 patients with isthmic lumbar spondylolisthesis(IS) and 42 patients with degenerative lumbar spondylolisthesis(DS) were included in this study, which ranged from January 2012 to July 2014. There were 15 males and 27 females with the age between 33 and 66 years in isthmic group, 15 males and 27 females with the age between 44 and 67 years in degenerative group. There was no significant difference in Oswestry disability index(ODI) score and Japanese orthopaedic association(JOA) score and the body mass index(BMI) between degenerative and isthmic lumbar spondylolisthesis patients. All parameters were evaluated on whole spine X-rays in a standing position. The following parameters were measured: slip percentage(SP), pelvic incidence(PI), sacral slope(SS), pelvic tilt(PT), cervical-thoracic angle(CTA), thoracic-lumbar angle (TLA), lumbar-sacral angle(LSA), full balance integrated(FBI). The Student’s test was employed for analysis differences in non-categorical variables between the two groups. Pearson correlation coefficient was used to analysis the association between all the sagittal parameters and slip percentage. Results: The slip percentage(SP) in IS group was 28.2°±9.6°, which was greater than the DS group(16.7°±8.3°,P<0.05). But the pelvic parameters, the spine- pelvis curve harmonious angle and the FBI showed no significant differences between the two groups(P> 0.05). According to the section of slip and the degree of slip, the two groups of patients were divided into four subgroups, we found there were also no significant differences of the parameters between the two groups. There were significantly correlation of PI, PT, LSA with SP in IS group(P<0.05), and PI, LSA with SP in DS group(P<0.05).Conclusions: In the case of BMI and clinical symptoms of the same, the degree of slippage in IS group is greater than the DS group, but the pelvic morphology, adjacent curves of the spine and full balance of torso are similar.
