Objective: To enhance the quality control of the pre-examination phase and improve the quality of clinical laboratory,we developed innovative, practical, efficient, easy, intuitive and interesting screen medium of specimen collection instruction. Methods: Investigating the samples’ unqualified rates at home and overseas: Investigate the samples’ unqualified rates and reasons in the pre-examination phase from the journals, dissertations and conference papers dated from January 2000 to December 2013. Collecting the self-collected samples’ unqualified rates and reasons at Affiliated Hospital of Luzhou Medical College: Inquired the laboratory information system(LIS), fulfilled a questionnaire survey and summarized the unqualified samples from the lab workers’ manual registration to inquire the unqualified rates and reasons of self-collection feces, urine and sputum at the Laboratory of Affiliated Hospital of Luzhou Medical College from January to June 2014. Made the samples’ collection instruction videos: Wrote the scripts of samples’ collection methods on the feces, urine and sputum and used adobe Flash CS5.0 software to make the samples’ collection instruction videos, which could be played repeatedly at proper places in hospital to guide the patients and their relatives on how to collect samples, Assess the effect ofself-collected guiding videos: after sending a questionnaire survey to the subjects and their relatives, collecting the unqualified rates and reasons of self-collected feces, urine and sputum from February-April 2015, using the χ2 test and Fisher exact probability test to compare the unqualified rates and reasons before and after the videos play, in order to evaluating the videos running effect. Results: The literatures reported the total rates of unqualified specimen were 0.12%-1.25%; the unqualified feces samples were 0.03%-15.29% which the main unqualified reasons were submit timeout, container error, fuzzy bar code and no sample; the unqualified urine samples were 0.01%-9.60% which the main unqualified reasons were insufficient the amount of sample, submit timeout and sample pollution and the unqualified sputum were 2.60%-76.00% which the main unqualified reasons were non-standard sputum and sample pollution. But at abroad, the unqualified rate of urine samples was 3.19%, the unqualified sputum sample is 5.90%-66.00% which the main unqualified reasons were non-standard sputum. After the videos play, the unqualified rate of feces routine specimen in our laboratory has fallen from 1.74% to 0.43%, which was statistically significant(χ2=158.83;P<0.05); the unqualified rate of feces cultivation specimen has fallen from 7.77% to 1.30%, which was statistically significant(χ2=23.94;P<0.05); the unqualified rate of urine routine samples has fallen from 2.42% to 0.53%, which was statistically significant(χ2=457.98; P<0.05); the unqualified rate of urine cultivation specimen has fallen from 13.56% to 6.75%, which was statistically significant(χ2=43.39;P< 0.05); the unqualified rate on sputum cultivation specimen has fallen from 30.20% to 6.79%, which was statistically significant(χ2=874.14;P<0.05). Conclusions: Currently, the unqualified rates of samples were a large differences and abnormally high, especially the self-collected specimens,which had great impact on the quality of the pre-examination phase. The videos have novel, intuitive, practicable, feasible, acceptable, durable and efficient characteristic, which can better guide the patients how to collect feces, urine and sputum specimen, decrease the unqualified rate and improve the quality of pre-examination phase and the level of laboratory management. The videos have the large value of application and promotion, meanwhile, it is one of the very important quality control measures in pre-examination phase.
