| Objective:To explore the hospital merger of hospitalized patients with rheumatoid arthritis the clinical characteristics of cardiocerebrovascular.Methods:A retrospective analysis the clinical material of 53 cases of RA patients with Cardiovascular disease and 64 cases of RA patients with Cerebrovascular disease in the first affiliated hospital of Guangxi Medical University during January 2012 to December 2014.Extraction a control group in the ratio of 1:1 of 117 contemporary cases in the diagnosis of RA without Cardiovascular and Cerebrovascular disease randomly according to gender and age.According to different disease in patients were divided into groups RA with cardiovascular disease and RA with Cerebrovascular disease, RA with cardiovascular disease and RA, RA with Cerebrovascular disease and RA. Were used t test and chi-square the to compare the above data,the use of non-conditional Logistic regression analysis of factors RA with cardiocerebrovascular diseases.Results:IRA with cardiovascular disease (1)The same period of hospitalization inpatients with RA 806 cases,of which 53cases of Cardiovascular disease,48 cases of Crown heart disease,20 cases of unstable angina patients (44.44%),11 cases of asymptomatic coronary artery disease (28.89%),9 cases of myocardial infarction(20%), stable angina 5 cases (11.11%), 3 cases of Crown heart disease complicated with acute left ventricular failure (5.66%),3 cases of Dilated cardiomyopathy(5.66%),2 cases of acute heart failure (3.77%).the prevalence was 6.58%,Male 13 cases,40 cases,female sex ratio of 1:3.1;these patients were older than the ensemble in the same period (66.45±10.27vs.45±11.56);64 cases RA with Cerebrovascular disease:58 cases of Lacunar infarction (90.63%),Five cases of cerebral arterial reduction (7.81%),1 case of Lacunar infarction with cerebral hemorrhage (1.56%). the prevalence was 7.94%.Male 23 cases,41cases,female sex ratio of 1:1.78;these patients were older than the ensemble in the same period (70.7±9.44 vs 49.45±11.56).2.the clinical characteristics:(l)the Cardiovascular disease the starting performance:Chest tightness (84.91%) Palpitations (67.92%),Chest pain (43.4%),Gasp (13.21%),Dizziness (24.53%),Edema (9.43%),fatigue (41.51%), No clinical symptoms of cardiovascular disease (24.53%); (2)ECG:33 cases were examined with ECG,the changes of T wave (36.36%)ã€the changes of ST-T (27.27%)ã€the abnormal Q wave (21.21%); (3)Echocardiography:21 cases were examined with Ultrasonic, Displaying plump and apical ventricular systolic function weakened in resting state, considered coronary heart disease (33.33%); (4) Chest CT:38 cases were examined with CT,9 cases (23.68%), coronary atherosclerosis; (5)Coronary angiography:19 cases were examined with Coronary angiography with different degrees of coronary stenosis, five cases (26.32%), â…¡ grade stenosis, six cases (31.58%),â…¢ grade stenosis, eight cases (42.11%), â…£ grade stenosis; (6)the Cerebrovascular disease the starting performance:Dizziness (48.43%), headache (25%), nausea and vomiting (14.06%), limb weakness (62.5%), stool incontinence (15.63%), Blurred consciousness (20.31%), unclear articulation (25%), visual rotation (14.06%), tinnitus (9.38%), melena (4.69%), mouth askew (23.44%), sudden syncope (4.69%), dysphagia (6.25%), blurred vision and ghosting (20.31%);(7) Imaging examination:33 cases were examined with brain CT examination,32 cases of cerebral infarction(96.97%),18 cases were examined with brain MRI examination, Which are suggestive of cerebral infarction (100%);7 patients were examined with brain MRA, including 4 cases of cerebral arteriosclerosis and stenosis (57.14%),2cases of cerebral arteriosclerosis(28.57%),l case of normal cerebral artery(14.29%);7 patients were examined with brain CTA, including 4 cases of cerebral arteriosclerosis and stenosis(57.14%),2cases of cerebral arteriosclerosis(28.57%),1 case of normal cerebral artery(14.29%).3. comparison of the clinical data between RA with cardiovascular disease and RA with cerebrovascular disease. The group comparison:course of the disease, Rheumatoid arthritis, Low density lipoprotein and Uric Acid was statistically significant (P<0.05), no statistical significance (P> 0.05).4. comparison of the clinical data between RA with cardiovascular disease and RA。The group comparison:duration of globulin, Triglyceride, High-density lipoproteins, Uric Acid, Albumin, C-reactiveprotein, Anti-cyclic citrullinated peptide antibody was statistically significant (P<0.05), no statistical significance (P> 0.05). Through the unconditioned Logistic regression analysis suggests RA course and Uric Acid are risk factor for RA with cardiovascular disease.5. comparison of the clinical data between RA with cerebrovascular disease and RA.The group comparison:duration of globuli, Hemoglobin, Albumin, Triglyceride, Low density lipoprotein, Anti-cyclic citrullinated peptide antibody was statistically significant (P<0.05), no statistical significance (P> 0.05).Through the unconditioned Logistic regression analysis suggests RA course and Triglyceride are risk factor for RA with cerebrovascular disease.Conclusion:1.Our hospital inpatients RA with cardiovascular disease main include Crown heart disease and Dilated cardiomyopathy,the prevalence was 6.58%, the ratio of 1:3.1; RA with cardiovascular disease main include Lacunar infarction and cerebral arterial reduction,the prevalence was 7.94%, the ratio of 1:1.78.2.RA long course and high Uric Acid are risk factor for RA with cardiovascular disease; RA long course and high Triglyceride are risk factor for RA with cerebrovascular disease. |
