Objective:Arthroscopy downward adhesion release clinical observation of treatment of knee joint stiffness.Methods:In March 2013 to June 2014 in our department of adhesion under arthroscopy release treatment of patients with knee joint stiffness.There are 26 patients, including due to the lower femur fractures in nine cases, patellar fractures in three cases, a femoral shaft fracture and ipsilateral patellar fracture in two cases, tibial fractures in one case, Juga between tibial condyle fracture in two cases, a femoral shaft fracture and ipsilateral tibial plateau fracture in two cases, after anterior cruciate ligament reconstruction under arthroscopy in one case, before and after the cut after cruciate ligament reconstruction in one case, before and after the cruciate ligament under arthroscopy combined medial collateral ligament reconstruction surgery in three cases, before and after the cruciate ligament reconstruction under arthroscopy combined femoral condyle fracture in one case, and soft tissue laceration after surgery in two cases. With the arthroscopic surgery treatment knee joint stiffness in 26 cases, intraoperative release capsule on patellar, nest of adhesion between intermediate muscle and femoral condyle, and massage to ease, intraoperative bleeding completely. Postoperative day one ice and quadriceps muscle strength exercise rehabilitation treatment such as anti-inflammatory and detumescence. Through the scope of activities before and after operation, the comparison of VAS and HSS score to assess the curative effect.Results:Take the arthroscopic downward adhesion solution treatment knee joint stiffness in 26 cases, and functional exercise early lines of system recovery. Through the comparison of range, the observed activity before and after knee surgery. The mean duration of follow-up was 6 months. For preoperative VAS score(1.46±2.04), at the end of the follow-up for VAS score (0.38±0.80), postoperative pain compared with preoperative (t=3.89, P< 0.001). Preoperative HSS score was (58.08±12.07), at the end of the follow-up for HSS score (71.96±8.98), postoperative knee function compared with preoperative improved significantly (t=14.77, P< 0.001). There were significant difference between preoperative the average of knee flexion angles and postoperative follow-up 6 months the average of knee flexion angles (106.54±12.87)°, Compared with preoperative increase 57°(t=-10.72, P< 0.001)。one case of myositis ossificans, no postoperative incision infection, skin necrosis, patellar tendon rupture and other complications.Conclusions:Arthroscopic surgery of knee joint stiffness method has small trauma, fewer complications, clinical effect is good wait for an adv antage, the operation is safe.
