Objective :Discuss the rehabilitation evaluation after cochlear implantation in inner ear malformation children of the extreme sensorineural hearing and before learing to speak speech,compared results with the normal inner ear children before learning to speak and extremely severe sensorineural hearing,to evaluate the effect of these patients after cochlear implantation.To provide the reference data for cochlear implant and postoperative rehabilitation training.Methods: Since 2011, retrospectively analyzed preoperative imaging diagnosis of 40 patients with inner ear malformation as the group A,the large vestibular aqueduct 31 cases,Mondini abnormalities 5 cases, internal auditory canal stenosis 1 case, Total cavity deformity 3 cases. And choose 40 patients whose age and the implant age were same or similar with the normal inner ear of extremely severe sensorineural deafness children as the normal control group B, the effect of speech rehabilitation were compared between the two groups of the 6 months,12 months, 18 months, 24 months after operation. The chosed deaf children were all rehabilitation tratining at schools,family training is complementary. The questionnaire survey or telephone follow-up for parents or rehabilitation teachers were available 。 We use categories of auditory performance and speech intelligibility rating to evaluation of deaf children of auditory perception ability and the ability of speech production respectively after cochlear implantation, we detailed explanation to each respondent, make sure that the respondent fully understand the classification stangdard of SIR and the CAP. Also asked respondents have objective evaluation, should not be affected by subjective feelings and expectations statistical analysis and comparison were conducted. At the same time 40 cases of cochlear implants with inner ear malformation patients were divided into 4 groups according to the age, there were >=9 years old, 9 years old,3-6 years old and <3 years old. The better scores for CAP of provisions of auditory rehabilitation effect is greater than 7 points.The better for SIR scores of speech rehabilitation effect is five points.All children with preoperative audiological examination results were bilateral extremely severe sensorineural deafness. The set of imaging examination of inner malformation children all have different degress of inner ear malformation, the children of normal control group with ear anatomy structure. Suspected with developmental abnormalities, abnormal psychological behavior are not include in the scope of the study.All the children have the same surgeon surgery, NRT responded well after surgery. All the pationts leave hospital after one week, boot debugging after a month.Results: Above the survey,80 copies of valid questionnaires were received and follow-up.By statistics analysis the set of CAP and SIR score of inner ear malformation children who lost their hearing before learning to speak of group A after cochlear implantation, compareing with the inner ear anatomy is normal,which has no statistical significance between B(P>0.05.The younger children with cochlear implants, the better the results. As the extension of time,Cochlear implant postoperative effect is better than before 。 The proportion of patients with auditory rehabilitation effect is better performance for increased along with the decrease of age when implanted。CAP is greater than 7 points, respectively85%(11/13),70%(7/10),44 %(4/9),38%(3/8);Speech rehabilitation SIR scored than five points were respectively 54 %(7/13),50 %(5/10),44%(4/9),38%(3/8),But with the decrease of the implanted age trends,the effect increased.Conclusion :Although the types of the inner ear malformation is various, the individual also has difference, the formal rehabilitation training after artificial cochlear implantation, the postoperative rehabilitation effect and no obvious difference was found between inner ear malformation and the normal children. Cochlear implantation can help the inner ear malformation patients regain hearing and speech ability, help it to return to sound society.
