Expression Of MTDH, HIF-1α And TKTL1 In Laryngeal Carcinoma And Its Clinical Indication

Objective: To discuss the expression of Metadherin(MTDH), hypoxia- inducible factor-1 alpha(HIF-1α)and transketolase-like gene 1(TKTL1) in laryngeal carcinoma and adjacent tissue and its relationship with tumor clinicopathological features. The interaction of these three factors in laryngeal carcinoma development was also explored.The purpose of this study is to provide theoretical evidences for laryngeal carcinoma treatment.Methods: The expression of MTDH, TKTL1 and HIF-1α protein were detected in 30 cases of laryngeal carcinoma tissue and 20 cases of adjacent tissue(exceed 0.5 cm distance from the tumour edge)by the immunohisto- chemical SP(streptavidin perotridase) method.The relationship among these three factors and their clinicopathological parameters were also analyzed.Results:1 The positive expression rate of MTDH in laryngeal carcinoma and adjacent tissues was 56.67% and 20.00% resectively, The positive expression rate of MTDH was 27.27% in well-differentiated laryngeal carcinoma and 73.68% in moderate and poor differentiated laryngeal carcinoma, The positive expression in tumour tissues with lymph node metastasis and without lymph node metastasis was 84.62 % and 35.29%,the difference of this three groups were statistical significance(P <0.05), The positive expression of MTDH inⅠ~Ⅱ stage and Ⅲ~Ⅳ stage was 50.00 % and 61.11%, the expression of MTDH was 38.46% in supraglottic laryngeal carcinoma and 70.59% in glottis laryngeal carcinoma, the expression of MTDH in tumour tissue was 60.00% in the patients less than 60 years old and 55.00% in the patients not less than 60 years old, the positive rate of MTDH was 59.10% in persons who smoked no less than twenty cigrattes per day and 50.00% in persons who smoked less than twenty cigrattes per day, all of this four groups had no significant difference statistically(P>0.05).2 The positive expression rate of TKTL1 in laryngeal carcinoma tissue and adjacent tissue was 63.33% and 15.00% respectively, The positive expression rate of TKTL1 in well-differentiated laryngeal carcinoma was18.18% and 89.47% in moderate and poor differentiated laryngeal carcinoma, The positive expression in tumour tissues with lymph node metastasis and without lymph node metastasis was 79.62% and 35.29% respectively, The positive expression rate of TKTL1 in stage Ⅰ~Ⅱ laryngeal carcinoma was 25.00% and 72.22%in stage Ⅲ~Ⅳ,the differences of this four groups were statistically significance(P<0.05). But the positive expression rate of TKTL1 in supraglottic laryngeal carcinoma was 46.15% and 76.47% in glottis laryngeal carcinoma, in the patients less than 60 years old and not less than 60 years old was 40.00% and 60.00%, The positive expression rate of TKTL1 in persons who smoked no less than twenty cigrattes per day was 50.00% and 62.50% in persons who smoked less than twenty cigrattes per day,all of this three groups had no significant difference statistically(P>0.05).3 The positive expression rate of HIF-1α in laryngeal carcinoma tissue and adjacent tissue was 60.0%and 10.00% respectively, in well- differentiated laryngeal carcinoma was 18.18% and in moderate and poor differentiated laryngeal carcinoma was 84.21%,in tumour tissues with lymph node metastasis and without lymph node metastasis was 84.62% and 41.18% respectively, in stage Ⅰ~Ⅱ laryngeal carcinoma was 33.33% and in stage Ⅲ~Ⅳ laryngeal carcinoma was 77.78%,the differences of this four groups were statistically significance(P<0.05). But the positive expression rate of HIF-1α in supraglottic laryngeal carcinoma and in glottis laryngeal carcinoma was 69.23% and 52.91% respectively,the difference had no statistic significa- nce(P> 0.05), in the patients less than 60 years old and no less than 60 years old, the positive expression rate of HIF-1α was 80.00% and 50.00% respectively,the difference had no statistic significance(P> 0.05), the positive expression rate of HIF-1α was 63.64% in persons who smoked no less than twenty cigrattes per day and 50.00% in persons who smoked less than twenty cigrattes per day, the difference had no statistic significance(P> 0.05).4 The spearman’s Rank Correlation Analysis showed that the positive expression of MTDH and HIF-1α in laryngeal carcinoma had significant correlation(r = 0.384, P <0.05); the positive expression of TKTL1 and HIF-1α in laryngeal carcinoma had significant correlation(r = 0.508, P <0.05); But it had no significant correlation between MTDH and TKTL1 expression(r =-0.107, P> 0.05).Conclusions:1. The protein expression of MTDH, TKTL1 and HIF-1α was higher in laryngeal carcinoma than that in para-carcinoma tissues. The expression of MTDH, TKTL1 and HIF-1α were related to lymphatic metastasis, but were not related to patient age and smoking. The expression of TKTL1 and HIF-1α were associated with tumor clinical staging,while the expression of MTDH and HIF-1α protein were associated with the degree of pathological differentiation. It suggested that these three proteins may have close relationship with the occurrence,invasion and metastasis of the laryngeal carcinoma.2. Both MTDH and TKTL1 expression were positively correlated with HIF-1α expression, suggesting that the laryngeal tumor cells could adapt to hypoxic microenvironment by increasing the expression of MTDH and TKTL1.MTDH and TKTL1 may be expected to be clinical targets for laryngeal carcinoma treatment and it could offer theoretical evidence for the prognosis of the laryngeal carcinoma.
